摘要
目的分析疾病诊断相关分组(DRG)对医生行为的影响,进而规范医生行为,完善DRG支付方式改革。方法以“DRG”“DRGs”“疾病诊断相关分组”等为主题词,在中国知网、维普等数据库检索相关文献,借助NVivo 11.0软件对纳入的文献进行三级编码的提取,分析DRG对医生行为影响的特征;基于社会认知理论,从三级编码结果层面分析DRG对医生行为的影响机理;并通过构建行为模型和函数表达式的形式总结DRG对医生行为的影响。结果纳入19篇文献作为研究资料,共得到204个参考点,归属于20个三级节点、8个二级节点和3个一级节点,根据各级节点构建出DRG对医生行为影响的模型。结论NVivo软件可以进行节点建立和关键词语选取,呈现出DRG对医生行为影响的三级结构;借助行为理论有助于构建行为模型,并可针对性地提出规范医生行为的策略与措施。
Objective To analyze the effect of DRG on doctors'behavior,so as to standardize doctors'behavior and improve the reform of DRG payment method.Methods Using“DRG”,“DRGs”and“diagnosis related groups”as the main topics,relevant literatures were searched in databases such as CNKI and VIP,and the included literatures were extracted by three-level coding with the help of NVivo 11.0 software to analyze the characteristics of DRG's influence on doctors'behavior.Based on the social cognitive theory,the influence mechanism of DRG on doctors'behavior was analyzed from the level of three-level coding results.The influence of DRG on doctors'behavior was summarized by constructing behavioral models and functional expressions.Results Nineteen literatures were included as research data,and a total of 204 reference points were obtained,which belonged to 20 third-level nodes,8 second-level nodes and 3 first-level nodes.According to the nodes at all levels,a model of the influence of DRG on doctors'behavior was constructed.Conclusion NVivo software can establish nodes and select key words,showing the tertiary structure of the influence of DRG on doctors'behavior.With the help of behavioral theory,it is helpful to construct behavioral models and put forward targeted strategies and measures to standardize doctors'behaviors.
