摘要
高血压是一种中老年人常见的心血管内科疾病,若患者血压无法得到有效控制,可引发冠心病、脑卒中和心力衰竭等心脑血管疾病,严重威胁患者的身心健康与生命安全。在对高血压患者提供必要的药物治疗之外,有效的社区管理对高血压患者的病情控制有着非常重要的作用。社区健康管理能帮助高血压患者提高自我管理能力,养成良好的生活习惯,实现良好的血压控制。高血压患者接受社区健康管理干预,能够有效控制并发症的发生,对患者生活质量提高有着积极意义。目前,高血压患者的社区健康管理模式主要有综合防治模式、规范化管理模式和三级预防模式等。影响高血压患者接受社区健康管理的因素较多,导致社区健康管理效果不尽理想。为更好总结高血压患者参加社区健康管理的经验,提高社区健康管理的实践价值,本文对高血压患者的社区健康管理进展进行综述。
Hypertension is a common cardiovascular disease in middle-aged and elderly people.If the blood pressure of patients can not be effectively controlled,it can lead to coronary heart disease,stroke,heart failure and other cardiovascular and cerebrovascular diseases,which seriously threaten the physical and mental health and life safety of patients.In addition to providing essential drug treatment for patients with hypertension,effective community management plays a very important role in the disease control of patients with hypertension.Community health management can help patients with hypertension improve their self-management ability,develop good living habits and achieve good blood pressure control.The patients with hypertension receiving community health management intervention can effectively control the occurrence of complications,which has a positive significance to improve the quality of life of patients.At present,the community health management mode of patients with hypertension mainly includes comprehensive prevention and control mode,standardized management mode and three-level prevention mode.There are many factors that affect patients with hypertension to accept community health management,which leads to the unsatisfactory effect of community health management.In order to better summarize the experience of patients with hypertension in community health management and improve the practical value of community health management,this paper reviews the community health management progress of patients with hypertension.
