摘要
目的:探讨替莫唑胺联合显微镜下经鼻蝶窦垂体瘤切除术治疗垂体瘤的效果。方法:选取2020年4月-2022年4月佳木斯市中心医院收治的102例垂体瘤患者为对象,随机分为对照组、观察组,各51例。对照组给予显微镜下经鼻蝶窦垂体瘤切除术治疗,观察组在对照组基础上增加替莫唑胺治疗。综合评估两组临床疗效、激素水平、炎症因子水平、生活质量、治疗安全性。结果:观察组总有效率为92.16%,高于对照组的74.51%(P<0.05);术后3个月,观察组生长激素(GH)、胰岛素样生长因子1(IGF-1)、催乳素(PRL)均低于对照组(P<0.05);术后3个月,观察组p53蛋白、基质金属蛋白酶2(MMP-2)、Xklp2靶蛋白(TPX2)水平均低于对照组(P<0.05);术后3个月,观察组躯体健康、心理健康、情感角色、社会功能评分均高于对照组(P<0.05);观察组、对照组不良反应发生率分别为15.69%(8/51)、9.80%(5/51),差异无统计学意义(P>0.05)。结论:替莫唑胺联合显微镜下经鼻蝶窦垂体瘤切除术治疗垂体瘤效果显著,可抑制患者激素和炎症因子的表达,改善患者生活质量,治疗安全性良好。
Objective:To investigate the effect of Temozolomide combined with microscopical transsphenoidal pituitary tumor resection in the treatment of pituitary tumor.Method:A total of 102 patients with pituitary tumor who were admitted to Jiamusi Central Hospital from April 2020 to April 2022 were selected and randomly divided into a control group and an observation group,51 cases in each group.The control group was treated with microscopical transsphenoidal pituitary tumor resection,and the observation group was treated with Temozolomide on the basis of the control group.The clinical efficacy,hormone levels,inflammatory factors levels,quality of life and treatment safety of the two groups were comprehensively evaluated.Result:The total effective rate of 92.16%in the observation group was higher than 74.51%in the control group(P<0.05).3 months after operation,the growth hormone(GH),insulin-like growth factor 1(IGF-1),prolactin(PRL)in the observation group were lower than those in the control group(P<0.05).3 months after operation,the levels of p53 protein,matrix metalloproteinase 2(MMP-2)and Xklp2 target protein(TPX2)in the observation group were lower than those in the control group(P<0.05).3 months after operation,the scores of physical health,mental health,emotional role and social function in the observation group were higher than those in the control group(P<0.05).The incidence of adverse reactions in the observation group and the control group was 15.69%(8/51)and 9.80%(5/51)respectively,the difference was not statistically significant(P>0.05).Conclusion:Temozolomide combined with microscopical transsphenoidal pituitary tumor resection is effective in the treatment of pituitary tumor,which can inhibit the expression of hormones and inflammatory factors,improve the quality of life of patients and have good treatment safety.
