摘要
目的观察针刺联合Brandt-Daroff习服训练治疗中老年耳石症复位后残余头晕的临床疗效。方法将72例中老年耳石症复位后残余头晕患者按照随机摸球法分为2组,对照组36例给予甲磺酸倍他司汀片治疗,治疗组36例在对照组基础上加用针刺联合Brandt-Daroff习服训练治疗。2组均治疗2周后统计疗效,比较2组治疗后头晕持续时间情况,比较2组治疗前后眩晕障碍量表(DHI)评分及匹兹堡睡眠质量指数(PSQI)量表(包括睡眠效率、睡眠时间、睡眠质量)评分变化情况。结果治疗组总有效率97.22%(35/36),对照组总有效率83.33%(30/36),治疗组疗效优于对照组(P<0.05)。治疗组治疗后头晕持续时间(9.84±2.45)h,对照组治疗后头晕持续时间(14.32±2.87)h,治疗组头晕持续时间短于对照组(P<0.05)。与本组治疗前比较,2组治疗后DHI评分均降低(P<0.05),且治疗组治疗后DHI评分低于对照组(P<0.05)。治疗组治疗后PSQI睡眠效率、睡眠时间、睡眠质量评分均较治疗前降低(P<0.05),且均低于对照组治疗后(P<0.05);对照组PSQI睡眠效率、睡眠时间、睡眠质量评分治疗前后比较差异均无统计学意义(P>0.05)。结论针刺联合Brandt-Daroff习服训练治疗中老年耳石症复位后残余头晕疗效确切,可有效减少头晕持续时间,减轻头晕程度,从而能够进一步改善患者睡眠质量,提高生活质量,促进残余头晕消除。
Objective To observe the clinical efficacy of acupuncture combined with Brandt-Daroff training for middle-aged and elderly patients with otolith disease with residual dizziness after reduction.Methods Totally 72 middle-aged and elderly patients with otolith disease with residual dizziness after reduction were assigned into the treatment group(n=36)and the control group(n=36)according to the random touch ball method;apart from the betahistinemesilate tablets,the treatment group additionally received acupuncture combined with Brandt-Daroff training.A 2-week treatment was performed to compare dizziness duration,Dizziness Handicap Inventory(DHI)score,Pittsburgh Sleep Quality Index(PSQI)(sleep efficiency,sleep time,sleep quality).The curative effect was assessed.Results After treatment,the overall effective rate in the treatment group was significantly higher compared with the control group(97.22%[35/36]83.33%vs[30/36],P<0.05).The dizziness duration in the treatment group was significantly shorter than that in the control group([9.84±2.45]h vs[14.32±2.87]h,P<0.05).DHI scores in groups after treatment were significantly decreased(P<0.05),which were common in the treatment group compared with the control group(P<0.05).PSQI scores in the treatment group were significantly decreased(P<0.05),and which were significantly lower than those in the control group(P<0.05).None of the PSQI scores differed significantly in the control group before and after treatment(P>0.05).Conclusion For middle-aged and elderly patients with otolith disease with residual dizziness after reduction,acupuncture combined with Brandt-Daroff training has definite curative effect,which can effectively reduce the dizziness duration and relieve dizziness symptoms,thereby further improving the quality of sleep and life,and promoting the elimination of residual dizziness.
