摘要
目的观察紫外激光照射联合曲安奈德注射液封包治疗白癜风的临床疗效及安全性。方法回顾性选取2020年3月—2022年2月南华大学附属第二医院收治的白癜风患者78例,根据治疗方法分为参照组(n=39)和联合组(n=39)。患者均实施紫外激光照射治疗,参照组未实施其他治疗措施,联合组应用曲安奈德注射液封包治疗3个月。比较2组临床疗效,治疗前、治疗3个月后T淋巴细胞亚群(CD_(3)^(+)、CD_(4)^(+)、CD_(4)^(+)/CD_(8)^(+))、白斑个数、白斑面积及皮肤病生活质量指数(DLQI)评分,不良反应发生率及随访6个月疾病复发率。结果联合组治疗优良率为71.79%,高于参照组的48.72%(χ^(2)=4.336,P=0.037)。治疗3个月后,2组CD_(3)^(+)、CD_(4)^(+)、CD_(4)^(+)/CD_(8)^(+)高于治疗前,且联合组高于参照组(P<0.05)。治疗3个月后,2组白斑个数少于治疗前,白斑面积小于治疗前,DLQI评分低于治疗前,且联合组白斑个数少于参照组,白斑面积小于参照组,DLQI评分低于参照组(P<0.01)。联合组不良反应总发生率为7.69%,低于参照组的28.20%(χ^(2)=4.523,P=0.033);联合组随访6个月复发率为5.13%,低于参照组的20.51%(χ^(2)=4.129,P=0.042)。结论采用紫外激光照射联合曲安奈德注射液封包治疗白癜风的疗效确切,可增强患者的免疫功能,有效改善白斑症状,提高患者的生活质量,且可降低不良反应及疾病复发风险。
Objective To observe the clinical efficacy and safety of UV laser irradiation combined with trimethoprim injection encapsulation in the treatment of vitiligo.Methods A total of 78 cases of patients with vitiligo treated in Nanhua University Second Affiliated Hospital from March 2020 to February 2022 were selected for the study,and they were divided into the reference group(n=39)and the combined group(n=39)according to treatment method.Patients were treated with UV laser irradiation,and the reference group did not take other measures,and the combined group was given tretinoin injection package for 3 months.Clincial effect,T lymphocyte subsets(CD_(3)^(+),CD_(4)^(+),CD_(4)^(+)/CD_(8)^(+)),number and area of white spots,DLQI score before and after 3 months of treatment,incidence of adverse reactions and disease recurrence rate after followed-up for 6 months were compared between the two groups.Results Excellent rate of the combined group was 71.79%,was higher than 48.72%of the reference group(χ^(2)=4.336,P=0.037).After 3 months of treatment,CD_(3)^(+),CD_(4)^(+)and CD_(4)^(+)/CD_(8)^(+)of the two groups were all statistically significant higher than those before treatment,and those of the combined group were higher than reference group(P<0.05).Compared with those before treatment,number of white spots of the two groups after 3 months of treatment was less,white spot area was smaller,and DLQI score was lower,and the above indexes of the combined group were better than those of the reference group(P<0.01).Incidence of adverse reactions of the combined group was 7.69%,which was lower than 28.20%of the reference group(χ^(2)=4.523,P=0.033);disease recurrence rate after followed-up for 6 months of the combined group was 5.13%,which was lower than 20.51%of the reference group(χ^(2)=4.129,P=0.042).Conclusion The combined of UV laser irradiation and tretinoin injection encapsulation has satisfactory efficacy for vitiligo,can positively affect the immune function of patients,effectively improve the symptoms of white spots and improve the quality of life,and reduce the risk of adverse reactions and disease recurrence.
