摘要
不宁腿综合征(restless leg syndrome,RLS)是临床常见的神经内科感觉-运动障碍性疾病。从“血脉理论”角度分析RLS的中医病机与辨治,认为血脉失和为本病的核心病机;营阴亏虚、血脉不充致肢体筋脉肌肤失养或痰瘀内生,寒湿痹阻致血脉瘀滞、肢体筋脉痹阻是其病机关键。治疗上,根据疾病病机之虚实以通补血脉为治疗关键,临床选用芍药甘草汤合四物汤滋阴补血以充养血脉,血府逐瘀汤活血化瘀、二陈汤化痰祛湿、桂枝汤散寒湿以通利血脉,并注重疏肝、调情志、安神以形神同调。对于特定人群,根据其生理特点,用药宜灵活加减。
Restless leg syndrome is a common sensory-motor disorder in neurology.Based on blood vessel theory,we think that the irregularity of blood vessels contributes to the core pathogenesis,and holds that the disease is due to:malnourishment of the limbs,tendons,muscles and skins(caused by ying-nutrient and blood deficiency),or stasis in the vessels and impediment of tendons and vessels(caused by both the engendered phlegm and blood stasis,and the cold dampness impediment).Thus in treatment,according to the deficiency or excess,we tonify or dredge blood vessels.Shaoyao Gancao Decoction combined with Siwu Decoction is selected to nourish yin and blood to tonify vessels,Xuefu Zhuyu Decoction to circulate blood and transform stasis,Erchen Decoction to transform phlegm and eliminate dampness,Guizhi Decoction to disperse cold and dampness for blood circulation.Meanwhile,we soothe the liver,regulate emotion and tranquilize the mind so as to adjust both the body and mind.As for specific groups of people,the medication should be adjusted according to their physiological characteristics.
