摘要
目的探讨改良腹腔镜下胆总管切开取石+T管引流术(LCTD)治疗胆道结石的临床应用价值。方法回顾性分析2016年6月至2021年12月在本院行胆总管结石手术患者120例。分为A组(改良LCTD组)、B组腹腔镜下胆总管切开取石一期缝合术(LBEPS)组、C组(传统LCTD组)。结果A组手术前后AKP下降速度要高于B、C组(P<0.05);A组术后发生胆漏、发热并发症例数要少于B、C组(P<0.05)。A组住院时间要短于C组(P<0.05);A组每日胆汁流量要少于C组(P<0.05)。结论腹腔镜、胆道镜、胃镜三镜联合的改良LCTD手术具有安全、有效、快捷、微创的特点,是治疗胆道结石的有效术式,适合临床推广。
Objective To evaluate the clinical value of modified laparoscopic cholecystectomy and T tube drainage(LCTD)in the treatment of biliary calculi.Methods A total of 120 patients who underwent choledocholithiasis surgery in the First People's Hospital of Fuyang District,Hangzhou from June 1,2016 to December 30,2021 were retrospectively analyzed.They were divided into the group A(modified LCTD group),the group B(LBEPS group)and the group C(traditional LCTD group).Results Alkaline phosphatase(AKP)decreased faster in the group A than in the group B and C before and after the operation.The number of postoperative biliary leakage and fever complications in the group A was less than that in the group B and C.The hospitalization time of the group A was shorter than that of the group C.The daily bile flow in the group A was less than that in the group C.The P value was all<0.05.Conclusion Modified LCTD combined with laparoscope,choledochoscope and gastroscope is safe,effective,fast,comfortable and minimally invasive.It is an effective operation for the treatment of biliary calculi and suitable for clinical promotion.
