摘要
胰十二指肠切除术已成为外科治疗胰头、壶腹部周围肿瘤等良恶性疾病的标准术式,该手术切除器官范围广、吻合难度大,是肝胆胰腺外科最复杂手术之一。由于壶腹部周围肿瘤常常压迫胆管引起梗阻性黄疸,可导致肝功能障碍、凝血功能异常、细菌移位继发胆管炎等,使患者术后并发症风险增高。近年来,对于胰十二指肠切除术前采用何种减黄方式存在争论。本文就不同减黄方式选择对胰十二指肠切除术后并发症的影响进行综述。
Pancreaticoduodenectomy has become the standard surgical procedure for benign and malignant diseases such as tumors of the pancreatic head and periampullary tumors.It is one of the most complex operations in hepatobiliary and pancreatic surgery because of its wide range of resection and difficult anastomosis.Since the periampullary tumor often compresses the biliary tract and causes obstructive jaundice,it can lead to liver dysfunction,abnormal coagulation function,bacterial translocation and secondary cholangitis,which increases the risk of postoperative complications.In recent years,there are controversies about the method of preoperative biliary drainage for patients undergoing pancreaticoduodenectomy.This article reviews the effect of different biliary drainage methods on postoperative complications after pancreaticoduodenectomy.
