摘要
目的 探讨胃肠间质瘤(GIST)不同中医证型的免疫组化因子表达及临床意义。方法 选取2019年2月至2021年10月于中山市人民医院就诊的胃肠道间质瘤患者60例作为研究对象,收集患者年龄、性别、临床特征、中医证型等资料,检测GIST组织C-kit原癌基因蛋白117(CD117)、CD34、Ki-67表达情况进行检测,比较不同中医证型CD117、CD34、Ki-67的表达情况,分析不同中医证型GIST组织CD117、CD34、Ki-67的相关性。结果 纳入的60例GIST患者中,GIST组织CD117、CD34、Ki-67阳性率分别为90.00%、81.67%、70.00%。不同肿瘤直径、危险程度患者Ki-67阳性表达率及不同中医证型患者CD117、CD34、Ki-67阳性表达率差异有统计学意义(P <0.05);经Spearman系数检验,GIST患者各中医证型比例分别与CD117呈正相关、与CD34、Ki-67阳性表达呈负相关(P <0.05)。结论 检测CD117、CD34、Ki-67的表达情况对GIST患者中医证型的诊断与鉴别具有重要意义,Ki-67阳性表达与肿瘤直径、危险程度有关,可作为评估GIST预后的一项指标。
Objective To investigate the expression and clinical significance of immunohistochemical factors in different traditional Chinese medicine(TCM)syndrome types of gastrointestinal stromal tumors(GIST).Methods A total of 60 GIST patients attended Zhongshan City People’s Hospital from February 2019 to October 2021 were selected as study subjects,and patients’age,gender,clinical features,TCM syndrome types and other related data were collected.The C-kit proto-oncogene protein(CD117),CD34,and Ki-67 were detected for expression in GIST tissues,and compared for their expressions in different TCM syndrome types.In addition,the correlations of CD117,CD34 and Ki-67 in GIST tissues of different TCM syndrome types were analyzed.Results Among the 60 GIST patients included,the positive rates of CD117,CD34 and Ki-67 in GIST tissues were 90.00%,81.67%and 70.00%,respectively.There were statistically significant differences in the Ki-67 positive rates among patients with different tumor diameters and risk levels,and in the CD117,CD34 and Ki-67 positive rates among patients with different TCM syndrome types(P<0.05).Spearman’s coefficient test showed that the proportion of each TCM syndrome type in GIST patients was positively correlated with CD117 and negatively correlated with positive CD34 and Ki-67 expressions(P<0.05).Conclusion The detection of CD117,CD34 and Ki-67 expressions is of significance for the diagnosis and identification of TCM syndrome types in GIST patients.And the positive Ki-67 expression is related to tumor diameter and risk level,which can be used as an index to evaluate the prognosis of GIST.
