摘要
目的观察基于“经筋理论”微创经筋针刀镜治疗痛风性膝关节炎临床疗效。方法将110例痛风性膝关节炎患者按照随机数字表法分为2组,对照组55例予常规西药治疗,治疗组55例予微创经筋针刀镜治疗。治疗2周后比较2组治疗前后膝关节Lysholm评分、疼痛视觉模拟评分(VAS)、血清白细胞介素8(IL-8)、IL-6、C反应蛋白(CRP)水平、血尿酸(UA)和红细胞沉降率(ESR)水平,并统计2组疗效。结果治疗组总有效率96.4%(53/55),对照组总有效率76.4%(42/55),治疗组临床疗效优于对照组(P<0.05)。2组治疗后Lysholm评分均较本组治疗前升高(P<0.05),疼痛VAS均降低(P<0.05),且治疗组治疗后Lysholm评分高于对照组(P<0.05),疼痛VAS低于对照组(P<0.05)。2组治疗后血清IL-8、IL-6、CRP水平均较本组治疗前降低(P<0.05),且治疗组治疗后均低于对照组(P<0.05)。2组治疗后UA、ESR水平均较本组治疗前降低(P<0.05),且治疗组治疗后均低于对照组(P<0.05)。结论微创经筋针刀镜治疗痛风性膝关节炎效果理想,可有效降低减轻疼痛,改善膝关节功能,降低CRP等炎症相关因子水平。
Objective To observe the clinical efficacy of minimally invasive needle knife mirror for gouty arthritis(GA)based on Meridian Sinew Theory.Methods Totally 110 GA patients were randomly assigned to the control group(n=55)and the treatment group(n=55),in which they were treated with conventional Western medicine and minimally invasive needle knife mirror,respectively.Treated for 2 weeks,the aim was to compare knee joint Lysholm score,visual analogue scale(VAS)for pain,serum interleukin-8(IL-8),IL-6,C-reactive protein(CRP),serum uric acid(UA),erythrocyte sedimentation rate(ESR).The curative effect was assessed.Results The overall effective rate in the treatment group was better than that in the control group(96.4%[53/55]vs 76.4%[42/55],[P<0.05],respectively).After treatment,Lysholm score in groups was increased(P<0.05),while VAS was decreased(P<0.05);higher Lysholm score,and lower VAS were detected in the treatment group than those in the control group(all P<0.05).The levels of IL-8,IL-6,CRP in groups were decreased after treatment(P<0.05),which were decreased in the treatment group compared with the control group(P<0.05).UA and ESR in groups were decreased(P<0.05),the reduction in the treatment group was more common(P<0.05).Conclusion For GA patients,minimally invasive needle knife mirror has an ideal therapeutic effect,can effectively reduce the VAS score,improve the CRP level,and it should be further promoted and applied in clinic.
