摘要
目的探讨胰高血糖素样肽1受体(GLP-1R)基因多态性与糖脂代谢关系及对儿童单纯性肥胖(CSO)的影响。方法选取2020年2月至2022年2月在十堰市太和医院就诊的91例CSO患儿(CSO组)及同期91例非CSO健康儿童(对照组),比较两组血糖指标[空腹血糖(FPG)、糖化血红蛋白(HbA1c)]、血脂指标[低密度脂蛋白胆固醇(LDL-C)、总胆固醇(TC)、甘油三酯(TG)、高密度脂蛋白胆固醇(HDL-C)],GLP-1R rs10305420、GLP-1R rs3765467、GLP-1R rs6923761位点多态性,以及GLP-1R rs10305420、GLP-1R rs3765467、GLP-1R rs6923761位点不同基因型患儿糖脂代谢情况,并采用Logistic回归分析CSO的相关影响因素。结果CSO组的TG显著高于对照组(t=3.774,P<0.05),CSO组的HDL-C显著低于对照组(t=4.853,P<0.05)。CSO组的GLP-1R rs10305420、GLP-1R rs3765467位点基因型分布与对照组比较差异均有统计学意义(χ^(2)值分别为8.839、9.312,P<0.05)。GLP-1R rs10305420位点CC基因型患儿的HDL-C显著高于CT+TT基因型(t=6.627),GLP-1R rs3765467位点GG基因型患儿的TG显著低于GA+AA基因型(t=12.902),差异均有统计学意义(P<0.05)。TG(OR=8.818,95%CI:1.087~71.536)、HDL-C(OR=0.310,95%CI:0.125~0.769)、GLP-1R rs10305420位点携带等位基因T(OR=10.976,95%CI:2.496~48.262),GLP-1R rs3765467位点携带等位基因A(OR=9.243,95%CI:1.530~55.843)均与CSO发生有关(P<0.05)。结论GLP-1R rs10305420位点携带等位基因T可导致HDL-C降低,GLP-1R rs3765467位点携带等位基因A可导致TG升高,且以上两位点多态性改变均与CSO有关,这为CSO防治提供了一种遗传学机制和新的方法。
Objective To investigate the relationship between glucagon-like peptide-1 receptor(GLP-1R)gene polymorphism and glucolipid metabolism and their effects on childhood simple obesity(CSO).Methods Ninety-one children with CSO(the CSO group)who were treated in the Shiyan Taihe Hospital from February 2020 to February 2022 and 91 healthy children without CSO during the same period(the control group)were selected.The blood glucose indexes[fasting plasma glucose(FPG),glycosylated hemoglobin(HbA1 c)],lipid indexes[low-density lipoprotein cholesterol(LDL-C),total cholesterol(TC),triacylglycerol(TG),high-density lipoprotein cholesterol(HDL-C)],and GLP-1R rs10305420,GLP-1R rs3765467,GLP-1R rs6923761 locus polymorphisms were compared between the two groups.The glucolipid metabolism of patients with different genotypes of GLP-1R rs10305420,GLP-1R rs3765467,GLP-1R rs6923761 locus were compared.Logistic regression analysis was used to analyze the related influencing factors of CSO.Results The CSO group had significantly higher TG level(t=3.774,P<0.05)and significantly lower HDL-C level(t=4.853,P<0.05)than the control group.The differences of the GLP-1R rs10305420 and GLP-1R rs3765467 locus genotypes between the CSO group and the control group were statistically significant(χ^(2)=8.839 and 9.312,respectively,P<0.05),respectively.HDL-C level in children with CC genotype at GLP-1R rs10305420 was significantly higher than that in children with CT+TT genotypes(t=6.627),TG level in children with GG genotype at GLP-1R rs3765467 was significantly higher than that in children with GA+AA genotypes(t=12.902),and the differences were statistically significant(P<0.05).TG(OR=8.818,95%CI:1.087-71.536),HDL-C(OR=0.310,95%CI:0.125-0.769),GLP-1R rs10305420 locus carrying allele T(OR=10.976,95%CI:2.496-48.262),and GLP-1R rs3765467 locus carrying allele A(OR=9.243,95%CI:1.530-55.843)were all associated with CSO occurrence(P<0.05).Conclusion The GLP-1R rs10305420 locus carrying allele T can lead to lower HDL-C level and the GLP-1R rs3765467 locus carrying allele A can lead to higher TG level,and both of the above two loci polymorphic alterations are associated with CSO,which provides a genetic mechanism and a new approach for CSO prevention and treatment.
