糖尿病肾脏病肾阳虚证与线粒体损伤指标的相关性研究 被引量:5

摘要 目的:探讨糖尿病肾脏病(DKD)肾阳虚证与线粒体损伤的相关性。方法:选择符合诊断标准的DKD患者56例,糖尿病(DM)患者28例,采集所有受试者临床资料,依据中医肾阳虚证诊断标准将DKD患者分为肾阳虚证组与非肾阳虚证组,采用酶联免疫吸附法检测血清线粒体损伤指标三磷酸腺苷(ATP)、超氧化物歧化酶(SOD)、丙二醛(MDA),进行统计分析。结果:与DM组相比,DKD组血红蛋白、肾小球滤过率、总胆固醇水平明显更低,血肌酐、尿素氮水平明显更高;线粒体损伤指标ATP、SOD水平随DM病情发展呈降低趋势,MDA水平呈升高趋势;ATP与SOD呈正相关,与糖化血红蛋白呈负相关,MDA与血红蛋白、白蛋白、肾小球滤过率呈负相关,与尿素氮、血肌酐呈正相关。与DKD非肾阳虚证组相比,DKD肾阳虚证组血红蛋白、血清白蛋白、肾小球滤过率水平明显更低,尿素氮、血肌酐水平明显更高;线粒体损伤指标ATP、SOD水平明显更低,MDA水平明显更高;DKD肾阳虚证积分与ATP呈负相关,与MDA呈正相关。结论:随着DM患者肾功能下降,线粒体损伤程度加重,线粒体损伤可能参与了DM到DKD的发展。肾阳虚证积分与线粒体损伤指标MDA、ATP的表达存在一定的相关性。 Objective:To explore the correlation between Kidney-Yang deficiency syndrome and mitochondrial damage in diabetic kidney disease(DKD).Methods:The clinical data of 56 cases of DKD and 28 cases of diabetes mellitus(DM),which met the diagnostic criteria,were collected.According to the diagnostic criteria of TCM Kidney-Yang deficiency,the patients were divided into the Kidney-Yang deficiency group and the non-Kidney-Yang deficiency group,enzyme-linked immunosorbent assay(ELISA)was used to detect the serum levels of mitochondrial damage indicators of adenosine triphosphate(ATP),superoxide dismutase(SOD),and malondialdehyde(MDA)for statistical analysis.Results:Compared with those in the DM group,the levels of hemoglobin and glomerular filtration rate were significantly reduced,and the levels of serum creatinine and urea nitrogen were significantly increased in the DKD group;the mitochondrial injury indicators of ATP and SOD were decreased with the development of DM,and MDA level showed an increasing trend;ATP was positively correlated with SOD,but was negatively correlated with urea nitrogen and blood creatinine;MDA was negatively correlated with hemoglobin and glomerular filtration rate,but was positively correlated with urea nitrogen and serum creatinine.Compared with those in the DKD non-Kidney-Yang deficiency group,the levels of hemoglobin,serum albumin,and glomerular filtration rate were significantly decreased in the DKD Kidney-Yang deficiency group,whereas the levels of blood urea nitrogen and serum creatinine were significantly increased;the levels of mitochondrial damage indexes of ATP and SOD were significantly decreased,MDA level was significantly increased;the score of DKD Kidney-Yang deficiency syndrome was negatively correlated with ATP,but was positively correlated with MDA.Conclusion:With the decline of renal function in DM patients,the degree of mitochondrial damage is aggravated,and mitochondrial damage may be involved in the development of DM to DKD.There is a certain correlation between Kidney-Yang deficiency integral and mitochondrial injury indicators of MDA and ATP.
