摘要
老年痴呆,又称阿尔茨海默病(AD),已成为危害公共卫生的重大难题,从中医古籍挖掘对老年痴呆的认识与治法是当前中医研究亟待解决的问题。本研究以金元时期易水学派代表医家李杲所论治疗“健忘”的诸方为入手点,通过扎实有据的文献考据研究,对李杲“升补”思想进行了溯源,认为其源于《黄帝内经》,承于易水学派张元素,并形成了以“升补”思想指导处方用药的鲜明特点。对于健忘,李杲以脾胃不足、清阳不升为论,以升阳风药与补益脾胃之药合用,临床常以升麻、柴胡、防风、羌活与人参、甘草、黄芪等药进行治疗,而现代研究所证明的AD起于“肠道菌群失调”之说,为李杲之“升补”思想治疗AD提供了有力的佐证。
Alzheimer s disease(AD),has become a major problem endangering public health.Mining the understanding and treatment of AD from ancient TCM books is an urgent problem to be solved in the current TCM researches.Based on the prescriptions for the treatment of‘forgetfulness’put forward by LI Gao,a representative physician of Yi Shui school in Jin and Yuan Dynasties,this study traced his thought origin of‘ascending and tonifying’.It considered that it originated from Yellow Emperor s Inner Canon,inherited ZHANG Yuansu s Yi Shui school,and formed distinctive characteristics of prescriptions guided by the thought of‘ascending and tonifying’.Regarding forgetfulness,LI Gao took the deficiency of Spleen and Stomach failing in ascending clear Yang as the target,and the combination of wind medicine to raise Yang and the medicine for tonifying Spleen and Stomach was applied.Rhizoma Cimicifugae,Radix Bupleuri,Divaricata Saposhnikovia,Notopterygium Incisum,Panax Ginseng,Glycyrrhiza and Astragalus Membranaceus were clinically used.The idea of AD originated from‘alteration of intestinal flora-gastrointestinal dysfunction-central nervous inflammation’,which has been proved by modern researches,provides strong evidence for LI Gao s thought of‘ascending and tonifying’in the treatment of AD.
