摘要
目的 探讨心电瀑布图在动态心电图分析中对阵发性心房颤动诊断的价值。方法 选取临床确诊为阵发性心房颤动患者60例,行24 h动态心电图检查,分别用模块分析方法、散点图分析方法及心电瀑布图分析方法进行对比分析,并且回顾分析阵发性心房颤动典型病例,阵发性心房颤动在心电瀑布图中的特征,评估心电瀑布图在诊断阵发性心房颤动的效果。结果 心电瀑布图分析诊断准确率(98.33%)高于模块分析(76.67%)及心电散点图分析(90.00%),心电瀑布图分析漏诊率(1.67%)低于模块分析(23.33%)及电散点图分析(10.00%);心电瀑布图分析诊断阵发性心房颤动平均耗时(1.27±0.50)h,低于模块分析(2.63±0.41)h及心电散点图分析(1.89±0.51)h。结论 心电瀑布图能高效、准确分析诊断阵发性心房颤动,降低漏诊率。
Objective To exolore the value of electrocardiogram waterfall in the diagnosis of paroxysmal atrial fibrillation(PAF) in Holter monitor analysis.Methods 60 patients with PAF were selected and the Holter monitor examination was performed, the results were analyzed by module analysis method, scatter plot analysis method and electrocardiographic waterfall analysis method respectively.The typical cases of PAF were analyzed retrospectively.The characteristics of PAF in electrocardiographic waterfall diagram were used to evaluated the value of electrocardiographic cascades in the diagnosis of PAF.Results The diagnostic accuracy of electrocardiographic waterfall analysis(98.33%) was higher than that of module analysis(76.67%) and electrocardiographic scatter plot analysis(90.00%).The missed diagnostic rate of electrocardiographic waterfall analysis(1.67%) was lower than that of module analysis(23.33%) and electrocardiographic scatter plot analysis(10.00%).The mean time for the PAF by electrocardiographic waterfall cardiogram was(1.27±0.50)h, which was lower than that by module analysis(2.63±0.41)h and electrocardiographic scatter plot analysis(1.89±0.51)h.Conclusion The electrocardiographic waterfall can effectively and accurately diagnose PAF and reduce the rate of missed diagnosis.
