从“筋伤”论颈型颈椎病 被引量:14

Discussion of Cervical Type Cervical Spondylosis from the “Tendon Injury” Theory

摘要 “筋骨失衡、筋伤为先”的中医理念为颈型颈椎病的防治提供了新的思路。最初的“筋”指联系四肢百骸,遍布全身的经络,随着中医筋伤学及现代生物力学的发展,医家对“筋”所包含的物质结构及生理功能有了更全面的补充,并且对颈筋系统中各成分在颈椎动静力学平衡中扮演的角色有了更明确认知。颈筋系统作为筋肉、韧带、椎间盘等软组织结构及生理功能的高度概括,相互间统一协调,共同发挥其“束骨而利关节”的作用,以保持颈椎力学平衡,因此颈型颈椎病“筋伤”的发生非单一结构损伤,“为先”的思想不仅局限于筋肉劳损层面。基于此,作者通过总结归纳颈筋系统损伤机制和损伤状态下各结构之间的力学、化学因素变化及相互间影响,为中医“筋骨失衡、筋伤为先”思想防治颈型颈椎病提供更加明确的思路。 The TCM concept of“imbalance of tendons and bones,tendon injury first”provides a new way of thinking about the prevention and treatment of cervical type cervical spondylosis. Initially,the term “tendons”referred to the meridians and channels that connect all the limbs and bones throughout the body.The role played by the various components of the cervical tendon system in the dynamic and static balance of the cervical spine has been more clearly understood. The“cervical tendon”system is a high level summary of the structure and physiological function of soft tissues such as tendons,ligaments and intervertebral discs,which work in unison to“bind the bones and facilitate the joints”in order to maintain the mechanical equilibrium of the cervical spine,hence the cervical type cervical spondylosis. The occurrence of“tendon injury”is not a single structural injury,and the idea of "being first" is not limited to the level of tendon strain. Based on this,the author summarized the mechanism of injury to the cervical tendon system and the changes in the mechanical and chemical factors between the various structures in the state of injury and their interactions,providing a clearer idea for the prevention and treatment of cervical type cervical spondylosis based on the TCM concept of“imbalance of tendons and bones,tendon injury first”.
