摘要
目的探究全踝关节置换术(TAR)术后护理对患者并发症及功能康复的影响。方法选取2019年4月-2020年3月于大连市第三人民医院行TAR治疗的76例患者,依据随机数字表法分为对照组与观察组,各38例。对照组采用通用术后护理模式,观察组采用TAR术后护理模式,比较两组术后疼痛模拟视觉(VAS)评分、住院时间、术后并发症发生情况、踝关节功能[踝关节评分(Kofoed)、美国矫形外科足踝协会(AOFAS)踝-后足功能评分]、踝关节功能恢复效果。结果观察组术后VAS评分低于对照组,且住院时间短于对照组(P<0.05);观察组并发症发生率为5.26%,低于对照组的21.05%(P<0.05);两组术后3个月Kofoed、AOFAS评分高于术后3 d,且观察组高于对照组(P<0.05);观察组踝关节恢复优良率为76.32%,高于对照组的57.89%(P<0.05)。结论TAR术后护理可缓解术后疼痛,缩短住院时间,降低并发症发生风险,促进患者踝关节功能恢复,值得临床应用。
Objective To explore the effect of total ankle replacement(TAR)postoperative nursing on complications and functional rehabilitation of patients.Methods A total of 76 patients who underwent TAR in Dalian No.3 People’s Hospital from April 2019 to March 2020 were selected and divided into control group and observation group according to random number table method,with 38 cases in each group.The control group adopted the general postoperative nursing mode,and the observation group adopted the TAR postoperative nursing mode.The postoperative pain simulation visual(VAS)score,hospitalization time,postoperative complications,ankle joint function[ankle joint score(Kofoed),American Orthopedic Foot and Ankle Society(AOFAS)ankle-hindfoot function score]and ankle joint function recovery effect were compared between the two groups.Results The postoperative VAS score of the observation group was lower than that of the control group,and the hospitalization time was shorter than that of the control group(P<0.05).The incidence of complications in the observation group was 5.26%,which was lower than 21.05%in the control group(P<0.05).The Kofoed and AOFAS scores of the two groups at 3 months after operation were higher than those at 3 days after operation,and those in the observation group were higher than those in the control group(P<0.05).The excellent and good rate of ankle joint recovery in the observation group was 76.32%,which was higher than 57.89%in the control group(P<0.05).Conclusion TAR postoperative nursing can relieve postoperative pain,shorten hospitalization time,reduce the risk of complications,promote the recovery of ankle function,and is worthy of clinical application.
