摘要
目的探讨妊娠期糖尿病与血清铁蛋白和糖脂代谢的关系。方法回顾性分析2020年2月-2021年2月于惠州市第一妇幼保健院定期产科检查的247例孕妇的临床资料,根据妊娠期糖尿病(gestaional diabetes mellitus,GDM)诊断标准分为GDM组146例,正常对照组101例。比较两组孕妇的血清铁蛋白(serum ferritin,SF)、空腹血糖(fasting plasma glucose,FPG)、服糖后1h血糖(1-hour postprandial blood glucose,1hPG)、服糖后2h血糖(2-hour postprandial blood glucose,2hPG)、总胆固醇(total cholesterol,TC)、甘油三酯(riglyceride,TG)。采用Pearson相关分析方法分析SF与糖脂代谢的相关性。采用受试者工作特征曲线(ROC)评价SF对GDM的诊断价值。结果GDM组孕妇的FPG、1hPG、2hPG、TG和SF水平明显高于对照组,差异有统计学意义(P<0.05)。相关分析结果提示,SF与TG、FPG、1PG、2hPG呈正相关(P<0.05),而正常对照组则无相关性(P>0.05)。ROC曲线下面积(AUC)为0.879(P=0.000),评判孕妇发生GDM的敏感度为81.5%,特异性80.2%。结论孕中期高水平的铁蛋白与糖脂代谢紊乱密切相关,对GDM具有一定的诊断价值。
Objective To investigate the correlation between gestational diabetes mellitus and serum ferritin and glycolipid metabolism.Methods The clinical data of 247 pregnant women who had regular prenatal examinations in the Huizhou No.1 Maternal and Child Service Center from February 2020 to February 2021 were retrospectively analyzed.According to the diagnostic criteria of GDM,146 cases were divided into GDM group and 101 cases were normal control group.The two groups of pregnant women were compared for serum ferritin(SF),fasting glucose(FPG),one hour postprandial blood glucose(1 hPG),two hours postprandial blood glucose(2 hPG),total cholesterol(TC),triglycerides(TG).The correlation between SF and glucose and lipid metabolism was analyzed by Pearson correlation analysis.Using receiver operating characteristic curves to evaluate the diagnostic value of SF in GDM.Results Compared with the control group,the levels of FPG,1 hPG,2 hPG,TG and SF were significantly increased in the GDM group(P<0.05).But there was no correlation in the control group(P>0.05).The area under the ROC curve(AUC)was 0.879(P=0.000).The sensitivity and specificity of GDM in pregnant women were 81.5%and 80.2%.Conclusion High level of SF in the second trimester is closely related to dysfunction of glucose and lipid metabolism,which has certain diagnostic value for GDM.
