摘要
目的探究2011—2020年我国HIV感染者抗病毒治疗(ART)及时性的变化。方法选取2011—2020年国家艾滋病综合防治信息系统中新诊断的HIV感染者为研究对象。以我国ART标准调整的时间节点为依据,划分"2011—2013年"(治疗标准:CD4+T淋巴细胞计数≤350个/μL)、"2014—2015年"(治疗标准:CD4+T淋巴细胞计数≤500个/μL)、"2016—2020年"(治疗标准:全部HIV感染者均可接受ART)三个ART标准阶段,回顾性分析不同ART标准阶段ART及时性的变化。结果2011—2020年新诊断的HIV感染者共计1100135例,3个ART标准阶段启动治疗的中位时间分别为300、70和24 d。多因素Cox回归分析显示,与2011—2013年确诊的感染者相比,2014—2015年(HR=1.45,95%CI:1.44~1.46)和2016—2020年(HR=2.41,95%CI:2.40~2.42)确诊的感染者ART启动更快。结论2011—2020年我国HIV感染者ART治疗及时性得到显著提高。
Objective To investigate the changes in the timeliness of antiretroviral therapy(ART)among HIV-infected people in China from 2011 to 2020.Methods Newly diagnosed HIV-infected patients from the China’s HIV/AIDS Comprehensive Response Information Management System from 2011 to 2020 were selected.Based on the time point of ART standard adjustment in China,three ART standard periods were divided including 2011-2013(ART standard:CD4^(+)T cell counts≤350 cells/μL),2014-2015(ART standard:CD4^(+)T cell counts≤500 cells/μL),2016-2020(ART standard:treat all).The changes in the timeliness of ART in different ART standard periods were retrospectively analyzed.Results From 2011 to 2020,a total of 1100135 newly diagnosed HIV-infected patients were included,and the median times to initiation of ART in different ART standard periods were 300,70 and 24 days,respectively.Multivariate Cox regression analysis showed that patients diagnosed with HIV infection in 2014-2015(HR=1.45,95%CI:1.44-1.46)and 2016-2020(HR=2.41,95%CI:2.40-2.42)initiated ART more quickly than those diagnosed during 2011-2013.Conclusions From 2011 to 2020,the timeliness of ART has been significantly improved.
