河南省育龄妇女孕前及孕早期血清TSH和FT4水平的参考值范围 被引量:2

Reference value range of serum TSH and FT4 levels of women of childbearing age in pre-pregnancy and early pregnancy in Henan Province

摘要 目的:探讨河南省育龄妇女孕前及孕早期甲状腺功能指标的参考值范围。方法:选取河南省6个县区2016年10月至2017年12月参加国家免费孕前优生健康检查的406名育龄妇女为研究对象,分别采集其孕前和孕早期(8~12周)空腹静脉血。采用化学发光法检测血清促甲状腺激素(TSH)、游离甲状腺素(FT4)水平。结果:406名河南省育龄妇女孕前和孕早期血清TSH水平分别为2.04(0.63~5.25)和1.20(0.04~4.45) mIU/L,孕早期较孕前降低(P<0.001);孕前和孕早期血清FT4水平分别为12.33(9.89~16.60)和11.91(9.02~19.12) pmol/L,孕早期较孕前降低(P<0.001)。不同年龄、BMI、孕次和孕周育龄妇女孕早期血清TSH水平均较孕前降低(P<0.001)。与孕前比较,20~岁、26~岁、BMI正常、孕次为0、孕周为10~12的育龄妇女孕早期血清FT4水平降低(P<0.05)。结论:本研究为河南省育龄妇女孕前及孕早期血清甲状腺功能指标参考值的建立提供了参考。 Aim:To explore the reference value range of thyroid function indexes of women of childbearing age in pre-pregnancy and early pregnancy in Henan Province.Methods:A total of 406 women of childbearing age who participated in National Free Preconception Check-up Projects from October 2016 to December 2017 in 6 counties and districts of Henan Province were selected as the subjects, and their fasting venous blood was collected in the pre-pregnancy and early pregnancy(8-12 weeks).Serum thyroid stimulating hormone(TSH) and free tetraiodothyronine(FT4) were measured by chemiluminescence method.Results:Serum TSH levels of 406 women of childbearing age in Henan Province were 2.04(0.63-5.25) mIU/L and 1.20(0.04-4.45) mIU/L in pre-pregnancy and early pregnancy, respectively, which were lower in early pregnancy than those in pre-pregnancy(P<0.001).The levels of serum FT4 in pre-pregnancy and early pregnancy were 12.33(9.89-16.60) and 11.91(9.02-19.12) pmol/L,respectively, which were lower in early pregnancy than those in pre-pregnancy(P<0.001).Serum TSH levels of the women of childbearing age in early pregnancy of different age, BMI,pregnancy times and gestational weeks were lower than those of the women in pre-pregnancy(P<0.001).Compared with those of the women in pre-pregnancy, the serum FT4 levels of the women aged 20-years, 26-years, with normal BMI,0 pregnancy time and 10-12 gestational weeks decreased in the early pregnancy(P<0.05).Conclusion:This study provides a reference for the establishment of values range of serum thyroid function indexes of women of childbearing age in pre-pregnancy and early pregnancy in Henan Province.
