摘要
本研究报告1例妊娠晚期子痫前期并发急性心力衰竭病例。患者由于“停经37+2周,呼吸困难0.5 h,伴咳嗽,咳痰”入院,入院后诊断为妊娠晚期子痫前期并发急性心力衰竭。予以持续母胎监护、强心、利尿、抗感染、硫酸镁静滴、硝酸甘油控制血压等治疗,经会阴侧切、胎头吸引术助产下顺利分娩,产后继续予以纠正心力衰竭治疗,并给予抗凝、退奶等处理,患者左室射血分数(LVEF)有所提高,预后良好。妊娠晚期子痫前期并发急性心力衰竭病情凶险,对母婴危害大,应做到早期预防、诊治,以改善母婴结局。
A case of preeclampsia complicated with acute heart failure in late pregnancy is reported in this paper. The patient was admitted to our hospital because of "menopause for 37 weeks and 2 days and dyspnea for 0.5 h, complicated with cough and expectoration". After admission, she was diagnosed as preeclampsia complicated with acute heart failure in late pregnancy. Continuous maternal fetal monitoring, cardiotonic therapy, diuretic therapy, anti-infection therapy, intravenous drip of magnesium sulfate, nitroglycerin to control blood pressure and other treatments were given. The patient was successfully delivered by lateral perineal incision and fetal head suction. After delivery, she continued to be treated with heart failure correction,anticoagulation, milk withdrawal and other treatments. The left ventricular ejection fraction(LVEF) of the patient was improved, and the prognosis was good. Preeclampsia complicated with acute heart failure in late pregnancy is dangerous, which is harmful to mother and baby. Therefore, early prevention, diagnosis and treatment should be carried out to improve the maternal and infant outcome.
