摘要
目的:探讨双层探测器光谱CT单能量成像联合低流速低用量对比剂对肠系膜下动脉图像质量的影响。方法:分析70例在苏州永鼎医院行全腹部CT增强的患者图像,分为两组,A组为实验组,对比剂流速2.5-3.0ml/s,剂量60-70ml,B组为对照组,对比剂流速4.5-5.0ml/s,剂量90-100ml。利用星云工作站将A组图像重建出40keV、50keV、60keV、70keV、80keV、100keV单能量组六组图像,测量并比较A组、B组常规混合能量组和A组的六组单能量组图像肠系膜下动脉CT值、噪声(SD)、信噪比(SNR)、对比噪声比(CNR)的差异,运用五分法对肠系膜下动脉主观图像进行评价分析其差异。结果:A组常规混合能量组的CT值、SNR、CNR均小于B组常规混合能量组,Z=7.20、3.20、2.87,P<0.05,差异均有统计学意义。40keV组的CT值、SD、SNR和CNR显著高于其他单能量组和常规组(Z=200.84、140.27、85.78、193.84,P<0.001),A组40keV单能量组的CT值、SD和CNR大于B组常规混合能量组,差异有统计学意义(Z=6.88、4.55、5.66,P<0.001)。两位医师对A组常规混合能量组、B组常规混合能量组、40keV、50keV、60keV、70keV、80keV、100keV的主观图像质量评价一致性良好或较好(Kappa=0.787、0.815、0.763、0.752、0.761、0.739、0.834、0.721,P<0.001),且两位医师对A组40keV图像主观评分高于B组常规混合能量组(Z=6.66、6.69,P<0.001)。结论:40keV可以在较低的对比剂流速和用量下,提高肠系膜及其分支的显示能力,值得在临床推广。
Objective:To investigate the effect of dual-layer detector spectral CT single-energy imaging combined with low flow rate and low dose of contrast agent on the image quality of inferior mesenteric artery.Methods:The images of 70 patients who underwent whole abdominal CT enhancement in Suzhou Yongding Hospital were analyzed,and they were randomly divided into two groups.Group A was the experimental group,with a contrast medium flow rate of 2.5-3.0ml/s and a dose of 60ml-70ml,and group B was the control group,with the flow rate of the contrast agent was 4.5-5.0ml/s,and the dose was 90ml-100ml.Use the nebula workstation to reconstruct six groups of images of 40keV,50keV,60keV,70keV,80keV,and 100keV single energy group from group A images.The CT value,noise(SD),signal-to-noise ratio(SNR),and contrast noise ratio(CNR)of the inferior mesenteric artery of group A,group B conventional mixed energy group,and group A were measured and compared.The subjective images of the inferior mesenteric artery were evaluated and analyzed by the five-point method.Results:The CT value,SNR,and CNR of the conventional mixed energy group in group A were all lower than those in the conventional mixed energy group in group B,Z=7.20,3.20,2.87,P<0.05,the differences were statistically significant.The CT value,SD,SNR,and CNR of the 40keV group were significantly higher than those of other single-energy groups and conventional groups(Z=200.84,140.27,85.78,193.84,P<0.001).The CT value,SD,and CNR of 40keV single energy group in group A were higher than those of the conventional mixed energy group in group B,and the difference was statistically significant(z=6.88,4.55,5.66,P<0.001).The subjective image quality evaluation of group A conventional mixed energy group,group B conventional mixed energy group,40keV,50kev,60keV,70kev,80kev,100keV by the two doctors is consistent or better(kappa=0.787,0.815,0.763,0.752,0.761,0.739,0.834,0.721,P<0.001),and the subjective score of A group 40keV image is higher than that of group B conventional mixed energy group(z=6.66,6.69,P<0.001).Conclusion:40keV can improve the display ability of the mesentery and its branches at a lower contrast agent flow rate and dosage,which is worthy of clinical promotion.
