摘要
目的了解ICU护士对病人生命末期维持治疗态度及决策行为意愿现状并分析其影响因素。方法方便抽样法抽取泸州市6家三级医院综合ICU的130名护士开展问卷调查,回收有效问卷122份,调查内容包括一般资料和生命末期维持治疗态度及决策行为意愿。以多元线性逐步回归法分析ICU护士对生命末期维持治疗态度及决策行为意愿的影响因素。结果ICU护士对病人生命末期维持治疗态度条目均分为(3.20±0.44)分,决策行为意愿条目均分为(3.76±0.57)分。自己生命末期是否会选择不接受心肺复苏、是否知晓安宁疗护均为ICU护士对病人生命末期维持治疗态度的影响因素(P<0.05);是否听过生命末期可合法放弃心肺复苏、自己生命末期时是否会选择不接受心肺复苏均是ICU护士对病人生命末期医疗决策行为意愿的影响因素(P<0.001)。结论ICU护士对病人生命末期维持治疗态度趋于正向,决策行为意愿倾向于不接受CPR等维持治疗措施,而采取安宁疗护等缓和性医疗手段,ICU护士的生命末期决策行为意愿趋于正向,应积极提高护士促进病人生命末期合理临床决策的参与度,从而提高病人末期生命质量。
ObjectiveTo understand the current status of ICU nurses’attitude towards life-sustaining treatment and decisionmaking intentions for the end-of-life patients,and analyze its influencing factors.MethodsA convenient sampling method was used to select 130 ICU nurses from 6 tertiary hospitals in Luzhou,122 valid questionnaires were collected.The contents of the survey included general information,attitude towards end-of-life maintenance treatment and willingness to make decisions.Multiple linear stepwise regression was used to analyze the influencing factors of ICU nurses’attitude towards life-sustaining treatment and decision-making behavior intention at the end of life.ResultsThe average score of ICU nurses’attitude toward life-sustaining treatment was(3.20±0.44),and the average score of decision-making behavior intention was(3.76±0.57).Whether they choose not to accept CPR at the end of their lives and whether they know hospice care are the influencing factors of ICU nurses’attitude towards life-sustaining treatment at the end of their life(P<0.05);whether they have heard that they can legally give up CPR at the end of life and whether they will choose not to accept CPR at the end of life were the influencing factors of ICU nurses’willingness to make medical decisions at the end of life(P<0.001).ConclusionICU nurses maintained a positive attitude towards end-of-life life-sustaining treatment,and their decision-making behavior intentions tended not to accept CPR and other life-sustaining treatment measures,but to adopt palliative medical means such as hospice care.ICU nurses’end-of-life decision-making behavior intention tended to be positive.Nurses should actively improve their participation in promoting reasonable clinical decision-making at the end of life,so as to improve the quality of life of patients at the end of life.
