摘要
目的:观察芍药甘草汤联合刺络拔罐治疗对强直性脊柱炎患者脊柱活动功能及生活质量改善的临床疗效。方法:对符合纳入和排除标准的30例强直性脊柱炎患者采用内服、外治相结合的方法,口服“芍药甘草汤”并辨证论治,并于大椎穴、筋缩穴、阿是穴行刺络拔罐。观察治疗前及治疗后患者Bath强直性脊柱炎病情活动指标(Bath Ankylosing Spondylitis Disease Activity Index,BASDAI)及Bath强直性脊柱炎功能指数(Bath Ankylosing Spondylitis Unctional Index,BASFI)评分,并对比患者治疗前及治疗后的枕墙距、指地距、胸廓活动度和36项简明健康状况评价量表(36-item Short Form Health Assessment Scale,SF-36)评分的变化。结果:患者治疗前后BASDAI及BASFI指数及枕墙距、指地距、胸廓活动度、SF-评分的差异有统计学意义(P<0.05)。结论:芍药甘草汤可活血化瘀、行气通络、柔筋止痛;刺络拔罐可“祛瘀生新”。芍药甘草汤联合刺络拔罐放血有助于减轻强直性脊柱炎患者关节疼痛,改善患者脊柱的活动度,改善关节功能,减少西药的应用频次,从而减轻药物的不良反应,改善患者的生活质量。探索强直性脊柱炎患者中西结合治疗之路具有重要的社会意义。
Objective:To observe clinical effects of the Shaoyao Gancao decoction(芍药甘草汤)plus blood-letting puncture and cupping on spinal function and life quality of the ankylosing spondylitis patients.Methods:Thirty cases met the inclusion and exclusion were treated by TCM medicine oral administration plus external therapy.The Shaoyao Gancao decoction was given orally based on syndrome differentiation,and blood-letting puncture and cupping was applied at Dazhui point(DU14),Jinshuo point(DU8)and Ashi point.The scores of BASDAI and BASFI were observed before and after treatment,and the occipital-wall distance,fingerground distance,thoracic motion and SF-36 scores were compared before and after treatment.Results:The BASDAI and BASFI scores after treatment were significantly different from those before treatment.The occipital-wall distance,finger-floor distance,horacic mobility and SF-36 scores after treatment were significantly different from before treatent(P<0.05).Conclusion:The Shaoyao Gancao decoction can promote blood circulation and remove blood stasis,activate Qi(气)and dredge collaterals,and soften tendon and relieve pain.Bloodletting puncture and cupping can remove blood stasis and promote growth.The Shaoyao Gancao decoction plus blood-letting puncture and cupping can relieve joint pain of patients with ankylosing spondylitis,ameliorate spinal mobility,improve joint function,and reduce the medication of Western medicine,so as to alleviate drugs adverse reactions and improve the quality of life.It is of great social significance to explore the treatment methods of ankylosing spondylitis in integrative medicine.
