摘要
肝细胞癌(hepatocellular carcinoma,HCC)是肝脏常见恶性肿瘤,具有浸润性强、恶性程度高、预后差的特点。大部分HCC患者在初诊时发现即为晚期,难以采用根治性手术进行切除。因此,临床上常采用介入治疗、肝动脉血管栓塞术、全身系统放疗化疗、分子靶向治疗等多种治疗方式联合应用,但中位生存时间、无进展生存期不尽人意,效果不甚理想,且晚期HCC患者缺乏统一的有效治疗措施。近年来,伴随着生物治疗技术的发展,免疫治疗以其毒副作用小、自身特异性高的特点,已成为肿瘤治疗的重要方式。免疫治疗与靶向治疗、放疗化疗、介入治疗等联合应用,在晚期HCC患者治疗方面取得了很好的疗效。文章对近几年联合免疫治疗在晚期HCC患者的临床应用进行综述,以期为临床上HCC免疫治疗提供参考。
Hepatocellular carcinoma(HCC)is a common malignant tumor of the liver,characterized by strong invasion,high degree of malignancy and poor prognosis.But most of the HCC patients are found to be in an advanced stage at the initial diagnosis,and it is difficult to undergo radical surgery for resection.Therefore,the clinical intervention therapy,hepatic arterial chemoembolization,systemic chemotherapy and radiotherapy,molecular targeted therapy and other treatment methods are often combined in clinical practice.However,the median survival time and progression-free survival time are not satisfactory,and there is a lack of unified and effective treatment measures for advanced HCC patients.In recent years,with the development of bio-therapy technology,immunotherapy has become a key method for the treatment of tumors due to its characteristics of low toxicity and high self-specificity.The immunotherapy combined with targeted therapy,radiotherapy,chemotherapy and interventional therapy has achieved good efficacy in the treatment of advanced HCC patients.This review summarizes the clinical application of combined immunotherapy in patients with advanced HCC in recent years,in order to provide references for clinical immunotherapy of HCC immunotherapy.
