摘要
目的 探讨结直肠癌(CRC)组织中KRAS基因突变和肿瘤坏死因子α(TNF-α)表达的关系及临床意义。方法 收集2022年1月至2022年6月南京医科大学第一附属医院手术切除的CRC组织标本194例。采用扩增受阻突变体系-聚合酶链反应(ARMS-PCR)法检测KRAS突变;免疫组织化学法检测TNF-α在CRC中的表达情况,分析KRAS突变与TNF-α表达的关系以及两者与CRC临床病理特征的关系。结果 与KRAS野生CRC组织相比,KRAS突变的CRC多发于直肠,且肿块直径明显大于KRAS野生组织,差异有统计学意义(P<0.05);进一步研究发现,KRAS基因突变的CRC组织的TNF-α表达水平低于KRAS基因未突变的CRC组织,差异有统计学意义(P<0.05);与TNF-α阳性相比,TNF-α阴性CRC多发于直肠且肿瘤直径增加,差异有统计学意义(P<0.05)。结论 KRAS突变CRC组织的TNF-α表达水平降低,TNF-α表达可成为CRC患者的治疗的潜在靶点。
Objective To investigate the relationship between KRAS gene mutation and expression of tumor necrosis factor α(TNF-α) in colorectal cancer(CRC) tissues and their clinical significances. Methods CRC tissue samples were collected from 194 CRC patients in the First Affiliated Hospital of Nanjing Medical University from January 2022 to June 2022. KRAS mutation was detected by amplification refractory mutation system based polymerase chain reaction(ARMS-PCR). Immunohistochemistry was used to detect the expression of TNF-α in CRC tissues. Relationship between TNF-α expression and KRAS mutation in CRC as well as their association with clinicopathologic features of CRC were further analyzed. Results CRC tissues with KRAS mutation was more common in rectum than that in tissues with wild type KRAS, and the mass diameter was significantly larger in tissues with KRAS mutation than wild type KRAS(P<0.05). Further studies showed that the expression of TNF-α in CRC tumor tissues with KRAS mutation was significantly lower than that in CRC tissues without KRAS mutation(P<0.05). In addition, CRC was more common in rectum and tumor diameter was significantly increased in tissues with negative TNF-α compared with positive TNF-α(P<0.05). Conclusion The expression of TNF-α in CRC with KRAS mutation is significantly decreased. TNF-α expression can be a potential target for the treatment of CRC patients.
