摘要
目的 评估应用急救护理指南对提高急诊科消化内镜治疗护理人员急救操作水平的有效性研究。方法 采用准实验性研究设计。急救护理指南由南京医科大学第一附属医院根据国家标准等参考编纂。入组对象包括所有与接受消化道内镜检查的患者一起工作的护理人员(28名护士)和急诊科120名成年患者。通过使用三个标准工具进行数据收集:即前/后患者结构化访谈,护士上岗前结构化访谈和护士执业观察性检查表。结果 实施急救护理指南后,护士对胃镜检查患者的知识和行为得分均有显著提高(P<0.05)。实施指南前后,护士在一般准备、术前、术后、实习总分等项目上的知识与实践呈正相关,有显著性差异,护士的急救操作水平得到了显著提升。结论 实施急救护理指南后,护士对急诊科消化内镜检查患者的护理知识和行为有明显提高,改善了患者的预后。建议对在胃肠内镜检查病房工作的护理人员进行持续和充分的教育和培训,并定期和持续地评估护士的执业情况。
Objective To evaluate a study of the effectiveness of applying emergency care guidelines to improve the level of emergency practice of nursing staff treating gastrointestinal endoscopies in the emergency department.Methods This study used a quasi-experimental research design.The undergraduate department compiles the guidelines for emergency care according to national standards and other references.Enrollment included all nursing staff(28 nurses) working with patients undergoing gastrointestinal endoscopy and 120 adult patients in the emergency department.Data collection was performed by using three standard tools:namely,the pre/post patient structured interview,the nurse pre-service structured interview,and the nurse practitioner observational checklist.A quasiexperimental study design was used.A convenience sample was used;it included all nursing staff(28 nurses) working with patients undergoing gastrointestinal endoscopy and 120 adult patients in the emergency department.The study was conducted in the gastrointestinal endoscopy unit of the First Affiliated Hospital of Nanjing Medical University.Three instruments were used to collect data.A pre/post patient structured interview,a nurse pre-induction structured interview,and a nurse practice observational checklist.Results After implementing the emergency care guidelines,nurses’ knowledge and behavioral scores for gastroscopy patients improved significantly(P<0.05).There were a positive and significant difference between nurses’ knowledge and practice on general preparation,preoperative,postoperative and total practice scores before and after implementation of the guidelines,and nurses’ emergency practice improved significantly.Conclusion There was a significant improvement in nurses’ knowledge and behavior in caring for patients undergoing gastrointestinal endoscopy in the emergency department after implementing the emergency care guidelines.As well as improving the prognosis of patients.This study recommends ongoing and adequate education and training for nursing staff working in gastrointestinal endoscopy units and regular and continuing evaluation of nurses’ practice.
