摘要
胰腺癌是难治性消化系恶性肿瘤,具有早期确诊率低、恶性程度高的特点,中医药在胰腺癌综合治疗中处于重要地位。王瑞平老师从多年临床经验中总结出胰腺癌“脾虚肝郁”的核心病机,治疗上强调以健脾为第一要义,以肝脾同调为基本治则,并针对关键病理因素如湿、痰、瘀、毒建立化湿泄浊、化痰散结、化瘀消癥、抗癌解毒等具体治法。同时王老师认为辨治胰腺癌须在分期论治的基础上动态辨明患者整体正邪盛衰,将辨病论治与辨证论治有机结合,融入“孟河医派”学术思想,形成了个体诊疗、动态调治、用药醇正、和缓图之的诊疗特色,临床上取得良好疗效。现从病因病机、治法治则,处方特色等方面总结梳理了王瑞平教授临床治疗胰腺癌的经验,以期指导临床实践,为临床上治疗该病开拓新思路。
Pancreatic cancer is a refractory digestive malignant tumor,with the characteristics of low early diagnosis rate and high degree of malignancy.Traditional Chinese medicine occupies an important position in the comprehensive treatment of pancreatic cancer.WANG Ruiping summed up the core pathogenesis of pancreatic cancer“spleen deficiency and liver stagnation”from many years of clinical experience.In terms of treatment,it is emphasized that invigorating the spleen is the first principle,and harmonizing the liver and spleen is the basic treatment principle.In view of the key pathological factors such as dampness,phlegm,blood stasis,and poison,specific treatment methods are established,such as dissipating dampness and dispelling turbidity,dissolving phlegm and resolving messes,removing stasis and eliminating abdominal mass,anti-cancer and detoxification method.Meanwhile,WANG Ruiping believed that the differential treatment of pancreatic cancer should dynamically identify the exuberance and debilitation of healthy qi and the pathogenic qi on the basis of treatment by stages.She organically combined treatment based on disease differentiation and syndrome differentiation,integrated into the academic thought of“Menghe medical school”,and formed the characteristics of individual diagnosis and treatment,dynamic treatment,drug mellow,and gentle treatment.It has achieved good clinical effects.This paper summarized WANG Ruiping’s experience in the treatment of pancreatic cancer from the etiology and pathogenesis,therapeutic method and principles,and prescription characteristics to guide the clinical practice and open up new ideas for the clinical treatment of the disease.
