摘要
目的分析高血压左室肥厚伴左心功能衰竭患者应用心脏彩超诊断的临床价值。方法随机选取2019年4月-2020年4月本院收治的高血压左室肥厚伴左心力衰竭患者48例,将其命名为研究组,再选取同一时间段内在本院接受健康体检的健康人员48例作为对照,将其命名为对照组,两组受检者均接受了心脏彩超检查,对两组受检者的左室舒张末期内径(LVEDD)、左室收缩末期内径(LVESD)、左室短轴缩短分数(LVFS)、左心射血分数(LVEF)以及二尖瓣口舒张早期最大血流速度/二尖瓣环舒张早期最大运动速度(E/Ea)检测结果进行比较,对比不同级别心力衰竭患者的左室舒张末期内径(LVEDD)、左房内径(LAD)以及左心射血分数(LVEF)。结果研究组患者的LVESD、LVEDD、LVFS均明显高于对照组,LVEF、E/Ea均明显低于对照组(P<0.05)。研究组患者中,Ⅲ级心衰患者LVEDD、LAD明显高于Ⅱ级心衰竭患者,Ⅱ级心衰竭患者LVEDD、LAD明显高于Ⅰ级心衰竭患者,Ⅲ级心衰患者LVEF明显低于Ⅱ级心衰竭患者,Ⅱ级心衰竭患者LVEF明显低于Ⅰ级心衰竭患者(P<0.05)。结论高血压左室肥厚伴左心力衰竭患者应用心脏彩超进行诊断具有较高的应用价值,能够有效评估患者心力衰竭严重程度。
Objective To analyze the clinical value of color Doppler echocardiography in the diagnosis of hypertensive left ventricular hypertrophy with left heart failure.Methods Forty-eight hypertensive patients with left ventricular hypertrophy with left heart failure treated in our hospital from April 2019 to April 2020 were randomly selected and named as the study group,and 48 healthy people who underwent physical examination in our hospital at the same time were selected as the control group.the subjects in both groups were examined by cardiac color Doppler ultrasound.The left ventricular end-diastolic diameter(LVEDD),left ventricular end-systolic diameter(LVESD),left ventricular short-axis shortening fraction(LVFS),left ventricular ejection fraction(LVEF)and mitral annular diastolic peak velocity/mitral annular diastolic peak velocity(E/Ea)were compared between the two groups.Left ventricular end-diastolic diameter(LVEDD),left atrial diameter(LAD)and left ventricular ejection fraction(LVEF)were compared in patients with different grades of heart failure.Results The levels of LVESD,LVEDD and LVFS in the study group were significantly higher than those in the control group,while the levels of LVEF and E/Ea in the study group were significantly lower than those in the control group.I in the study group,LVEDD and LAD in patients with gradeⅢheart failure were significantly higher than those in patients with gradeⅡheart failure,LVEDD and LAD in patients with gradeⅡheart failure were significantly higher than those in patients with grade I heart failure,LVEF in patients with gradeⅢheart failure was significantly lower than that in patients with gradeⅡheart failure,and LVEF in patients with gradeⅡheart failure was significantly lower than that in patients with grade I heart failure.Conclusion Fhe application of color Doppler ultrasound in the diagnosis of hypertensive left ventricular hypertrophy with left heart failure has a high application value and can effectively evaluate the severity of heart failure.
