摘要
目的探讨胃息肉患者血清胃功能胃蛋白酶原Ⅰ(pepsinogenⅠ,PGⅠ)、胃蛋白酶原Ⅱ(pepsinogenⅡ,PGⅡ)、胃泌素-17(gastrin-17,G-17)水平的变化及与Hp感染性研究。方法选取2019年9月—2021年8月于北京燕化医院门诊及住院的胃息肉患者130例作为观察组,选取同期体检的118名健康者作为对照组,比较各组的PGⅠ、PGIⅡ、PGR及G-17水平变化,并进一步将130例胃息肉患者根据病理类型分组(增生性息肉64例、炎性息肉34例、胃底腺息肉22例、腺瘤性息肉10例),比较各组间PGⅠ、PGIⅡ、PGR及G-17水平变化、息肉发生部位与各组的Hp阳性率。结果观察组PGⅠ、PGIⅡ、G-17水平较高,差异有统计学意义(P<0.05)。增生性息肉组与炎性息肉组PGⅠ、PGIⅡ、G-17、PGR水平比较,差异无统计学意义(P>0.05);增生性息肉组的G-17水平高于胃底腺息肉组,差异有统计学意义(P<0.05);炎性息肉组的G-17水平高于胃底腺息肉组,差异有统计学意义(P<0.05);130例胃息肉Hp阳性率为46.2%;其中增生性息肉组Hp感染率为56.7%,明显高于胃底腺息肉Hp感染率13.3%,差异有统计学意义(χ^(2)=4.398,P<0.05);炎性息肉组HP感染率为31.7%,明显高于胃底腺息肉组,差异有统计学意义(χ^(2)=4.424,P<0.05);增生性息肉组与炎性息肉组Hp感染率比较,差异无统计学意义(P>0.05)。结论胃增生性息肉与炎症性息肉患者血清PGⅠ、PGIⅡ、G17水平较高,胃黏膜存在明显炎症刺激,Hp感染与增生性息肉及炎性息肉有相关性,具有临床指导意义。
Objective To investigate the changes of serum gastric function pepsin I(PGI),pepsinogen II(PGII),gastrin-17(G-17)levels and study on Hp infectivity in patients with gastric polyps.Methods One hundred and thirty patients with gastric polyps who were outpatients and inpatients of Beijing Yanhua Hospital from September 2019 to August 2021 were selected as the observation group,and 118 healthy individuals who were examined during the same period were selected as the control group.The changes of PGI,PGII,PGR and G-17 levels in each group were compared,and the 130 gastric polyps were further grouped according to pathological types(64 hyperplastic polyps,34 inflammatory polyps,22 fundic glandular polyps and 10 adenomatous polyps).The changes of PGI,PGII,PGR and G-17 levels,site of polyp were compared among the groups,and the Hp-positive rate of each group was counted.Results The levels of PGI,PGII and G-17 in gastric polyps were higher in the observation group,and the difference was statis-tically significant(P<0.05).There was no statistically significant difference between PGI,PGII,G-17,and PGR levels in hyperplastic polyps and inflammatory polyps(P>0.05).The G-17 level in the hyperplastic polyp group was higher than that in the fundic gland polyp group,and the difference was statistically significant(P<0.05).The G-17 level in the inflammatory polyp group was higher than that in the fundic gland polyp group,and the difference was statistically significant(P<0.05).130 gastric polyps had a positive Hp rate was 46.2%;among them,the Hp infection rate in the hyperplastic polyp group was 56.7%,which was significantly higher than that in the fundic gland polyp group was 13.3%,and the difference was statistically significant(χ^(2)=4.398,P<0.05).The HP infection rate in the inflammatory polyp group was 31.7%,which was significantly higher than that in the fundic gland polyp group,and the difference was statistically significant(χ^(2)=4.424,P<0.05).There was no statistically significant difference in the Hp infection rate between the hyperplastic polyp group and the inflammatory polyp group(P>0.05).Conclusion Patients with gastric hyperplastic polyps and inflammatory polyps had higher levels of serum PGI,PGII and G17,and there was significant inflammatory stimulation of gastric mucosa,and Hp infection was correlated with hyperplastic polyps and inflammatory polyps,which had clinical guideline significance.
