摘要
目的:探讨急性上消化道出血救治快速通道的临床效果。方法:分析2021年6月—2022年2月北部战区总医院启动急性上消化道出血救治快速通道后经急诊科收治的消化道出血患者的临床资料,并选取2020年9月—2021年5月未启用快速通道时救治的患者临床资料进行对比。结果:快速通道启用后急性上消化道出血患者首次内镜检查时间、止血时间、输血量、住院时间、治疗费用分别为(9.72±2.80)h、(9.76±2.83)h、200(0,400)mL、(6.18±1.34)d、(9237.40±3288.84)元,均低于快速通道启用前的(17.66±5.01)h、(18.11±5.99)h、400(0,600)mL、(7.77±1.72)d、(11 275.38±2749.34)元,通道启用后内镜检查率、手术率及病死率(98.33%,10.83%,0.83%)均优于启动前(67.34%,3.44%,6.03%),差异均有统计学意义(P<0.05)。结论:急性上消化道出血快速通道可提高医院急性上消化道出血救治能力,可推动急性消化道出血患者得到及时有效的救治。
Objective: To investigate the clinical effect of fast track in the treatment of acute upper gastrointestinal bleeding. Methods: The clinical data of patients with gastrointestinal bleeding, who were admitted to the emergency department of General Hospital of the Northern Theater Command, from June 2021 to February 2022 were compared with the clinical data of patients without fast-track treatment from September 2020 to May 2021. Results: After activation of the fast track, the time of first endoscopic examination, hemostasis time, blood transfusion volume, hospital stay, and treatment cost in patients with acute upper gastrointestinal bleeding after fast-track activation were(9.72±2.80)h,(9.76±2.83)h, 200(0, 400)mL,(6.18±1.34) d and(9237.40±3288.84) Yuan, respectively, which were all lower than those without fast-track activation[(17.66±5.01)h,(18.11±5.99)h, 400(0, 600)mL,(7.77±1.72)d,(11275.38±2749.34) Yuan], and the differences were statistically significant(P<0.05). Endoscopy rate, surgery rate, and mortality rate(98.33%, 10.83%, 0.83%) after channel activation were better than before initiation(67.34%, 3.44%, 6.03%), and the differences were statistically significant(P<0.05). Conclusion: The fast track of acute upper gastrointestinal bleeding can improve the hospital’s ability to treat acute upper gastrointestinal bleeding, and promote the timely and effective treatment of patients with acute gastrointestinal bleeding.
