摘要
目的探讨虚拟现实结合关节镜治疗累及前交叉韧带(ACL)止点的胫骨平台骨折的临床效果。方法选取2017年1月至2022年3月萍乡市第二人民医院骨科行手术治疗的50例累及ACL胫骨止点的胫骨平台骨折患者,以随机数字表法将其分为对照组与观察组,各25例。对照组患者采用常规手术方式,观察组患者给予虚拟现实结合关节镜治疗方法。比较两组患者的手术时间、切口大小、术后视觉模拟评分法(VAS)评分、Rasmussen胫骨髁部骨折膝关节功能评分、美国特种外科医院(HSS)膝关节评分及膝关节活动度(ROM),并评价该种治疗方法的临床使用价值。结果观察组患者的手术时间、手术切口大小短于对照组,VAS评分低于对照组,Rasmussen功能评分优良率、HSS膝关节评分优良率高于对照组,ROM高于对照组,差异均有统计学意义(P<0.05)。结论虚拟现实结合关节镜治疗累及前交叉韧带止点的胫骨平台骨折能够缩短手术时间,减小切口大小,抑制术后疼痛,促进关节功能及改善活动度。
Objective To investigate the clinical effect of virtual reality combined with arthroscopy in the treatment of tibial plateau fractures involving Anterior cruciate ligament(ACL)insertion.Methods From January 2017 to March 2022,50 patients with tibial plateau fracture involving ACL tibial insertion were selected for surgical treatment in the Department of Orthopedics of Pingxiang Second People's Hospital,they were divided into the control group and the observation group by random number table method,with 25 patients in each group.The patients in the control group were treated with conventional operation method,while the patients in the observation group were treated with virtual reality combined with arthroscopy operation method.The operation time,incision size,postoperative visual analogue scale(VAS)score,Rasmussen knee function score for tibial condylar fracture,the knee joint score of American Special Surgery Hospital(HSS)and the knee joint range of motion(ROM)of the two groups were compared,and the clinical application value of this treatment method was evaluated.Results The operation time and incision size in the observation group were shorter than those in the control group,the VAS score in the observation group was lower than that of the control group,the excellent and good rate of Rasmussen function score and the excellent and good rate of HSS knee score in the observation group were higher than those of the control group,the ROM in the observation group was higher than that of the control group,and the differences were statistical significant(P<0.05).Conclusion Virtual reality combined with arthroscopy in the treatment of tibial plateau fracture involving ACL insertion can shorten the operation time,reduce the incision size,inhibit postoperative pain,and promote the improvement of joint function and range of motion.
