摘要
目的基于文献数据挖掘中药治疗急性髓系白血病(acute myeloid leukemia,AML)的用药规律,为临床治疗AML提供药物组方参考。方法以中国知网、万方医学网、维普医学网收录的文献为数据来源,统计中医药治疗AML的方剂;对方剂所包含的中药使用频次、四气五味、归经以及药物归类分别进行统计;最后,通过SPSS Modeler 14.1、SPSS Statistics 19.0等软件对中药进行关联规则分析和系统聚类分析。结果在符合纳入标准的224首方剂中,使用频率最高的药物有黄芪、白花蛇舌草、当归、白术等;性味以寒温、甘苦为主;归经则以肝经、肺经、脾经以及肾经为主;功效分类发现补虚药、清热药使用频率最高,其次是活血化瘀药;关联规则分析发现的核心药对有黄芪-当归-白花蛇舌草、白花蛇舌草-半枝莲等;系统聚类分析发现高频药物可分为3大类:补虚药、清热药以及活血化瘀药。结论中医药临床治疗AML主要以补虚药和清热药为主,其次是活血化瘀药。临床治疗应从热、毒、虚、瘀四个方面着手,根据化疗的不同阶段进行辨证治疗。
Objective To explore the medication rules of traditional Chinese medicine(TCM)in the treatment of acute myeloid leukemia(AML)based on literature data mining,thus providing a reference for the selection of TCM administrations.Methods The literatures about TCM treatment of AML were collected from China National Knowledge Infrastructure(CNKI),WanFang Medical Network and Weipu Medical Network.Then,the frequency,four natures,five flavors,meridian tropism and classification of all herbs in the prescriptions were collected.Finally,systematic association and clustering analysis of the prescription herbs were carried out using SPSS Modeler14.1 and SPSS Statistics19.0.Results Among the 224 prescriptions that met the inclusion Huangqi(Astragali Radix),Baihuasheshecao(Hedyotis Diffusa),Danggui(Angelicae Sinensis Radix),Baizhu(Atractylodis Macrocephalae Rhizoma)were used most frequently.The taste was mainly cold,warm,sweet taste and bitter taste.The major meridian tropisms belonged to liver,lung,spleen and kidney.Tonic herbs and heat-clearing herbs were the most frequently used,followed by blood-invigorating and stasis-dissolving herbs.Association analysis showed that the core drug pairs include Huangqi(Astragali Radix)-Danggui(Angelicae Sinensis Radix)-Baihuasheshecao(Hedyotis Diffusa),Baihuasheshecao(Hedyotis Diffusa)-Banzhilian(Scutellariae Barbatae Herba),etc.The most frequently used herbs can be divided into three categories by systematic clustering,namely,drugs for tonic,heatclearing and promoting blood circulation and removing blood stasis.Conclusion In clinical treatment of AML with TCM,tonic and heat-clearing herbs were mainly adminstrated,followed by the use of blood-invigorating and stasis-dissolving herbs.Heat,toxin,deficiency and blood stasis should be considered and the treatment should based on syndrome discrimination according to different stages of chemotherapy.
