摘要
目的:观察地氟烷复合瑞芬太尼在初产妇剖宫产麻醉中的应用效果。方法:选取2019年2月至2020年12月该院收治的104例初产妇进行前瞻性研究,按照随机数字表法分为对照组和观察组各52例。对照组采用全凭静脉麻醉(丙泊酚+罗库溴铵+瑞芬太尼),观察组采用地氟烷复合瑞芬太尼麻醉,比较两组不同时间点[T_(0)(麻醉诱导前)、T_(1)(诱导后3 min)、T_(2)(手术结束时)]血流动力学指标[心率(HR)、血氧饱和度(SPO_(2))及平均动脉压(MAP)]水平、分娩时间指标(麻醉诱导至胎儿娩出、腹膜切开至子宫切开、子宫切开至胎儿娩出时间)水平、术后恢复指标(苏醒时间、拔管时间、恢复定向力时间)水平和新生儿Apgar评分。结果:T_(0)、T_(1)、T_(2)时,两组HR、SPO_(2)、MAP水平比较,差异均无统计学意义(P>0.05);两组术中麻醉诱导至胎儿娩出、腹膜切开至子宫切开、子宫切开至胎儿娩出时间比较,差异均无统计学意义(P>0.05);观察组麻醉清醒时间、拔管时间和恢复定向力时间均明显短于对照组,差异有统计学意义(P<0.05);分娩后1、5、10 min,两组新生儿Apgar评分比较,差异均无统计学意义(P>0.05)。结论:地氟烷复合瑞芬太尼应用于初产妇剖宫产麻醉中可缩短产妇麻醉清醒时间、拔管时间和恢复定向力时间,优于单纯静脉麻醉效果。
Objective:To observe application effects of Remifentanil combined with Desflurane in cesarean section anesthesia of primiparas.Methods:A prospective study was conducted on 104 primiparas admitted to the hospital from February 2019 to December 2020.They were divided into control group and observation group according to the random number table method,52 cases in each.The control group was anesthetized with Remifentanil,while the observation group was anesthetized with Desflurane combined with Remifentanil.The levels of hemodynamic indexes[heart rate(HR),oxygen saturation(SPO_(2))and mean arterial pressure(MAP)]at different time points[T_(0)(before anesthesia induction),T_(1)(3 min after induction),T_(2)(at the end of surgery)],the delivery time index levels(time from anesthesia induction to fetal delivery,time from peritoneal incision to uterine incision,time from uterine incision to fetal delivery),the postoperative recovery index levels(recovery time,extubation time,recovery time of orientation force),and the neonatal Apgar score were compared between the two groups.Results:At T_(0),T_(1) and T_(2),there were no significant differences in the levels of HR,SPO_(2) and MAP between the two groups(P>0.05).There were no significant differences in the time from anesthesia induction to fetal delivery,time from peritoneal incision to uterine incision,time from uterine incision to fetal delivery between the two groups(P>0.05).Further,the recovery time,the extubation time and the recovery time of orientation force in the observation group was significantly shorter than that in the control group,and the difference was statistically significant(P<0.05).At 1 min,5 min and 10 min after delivery,there was no significant difference in Apgar scores between the two groups(P>0.05).Conclusions:Desflurane combined with Remifentanil for cesarean section anesthesia in primiparas can shorten the recovery time,the extubation time and the recovery time of orientation force.Moreover,it is superior to single Remifentanil anesthesia.
