摘要
目的研究维生素D及甲泼尼龙琥珀酸钠联用对过敏性紫癜(henoch-schonlein purpura,HSP)患儿的预后效果及对细胞免疫功能的影响。方法选择2020年2月至2022年2月杭州市儿童医院收治的150例HSP患儿,采用随机数字表法分为观察组与对照组,每组各75例,对照组予静脉滴注甲泼尼龙琥珀酸钠治疗,观察组在对照组的基础上联合维生素D治疗,比较两组的临床疗效,症状消失时间及住院时间、细胞免疫功能、不良反应等。结果观察组患儿治疗后的总有效率明显高于对照组(P<0.05)。观察组患儿治疗后的皮疹、腹痛、关节肿痛消失时间及住院天数分别短于对照组(P<0.05)。治疗后观察组CD3+、CD4+及自然杀伤细胞水平分别高于对照组(P<0.05)。两组不良反应比较,差异无统计学意义(P>0.05)。结论维生素D及甲泼尼龙琥珀酸钠联合应用治疗HSP患儿预后效果显著,有利于提高患儿的细胞免疫功能,安全性良好。
Objective To study the prognostic effect of vitamin D and methylprednisolone sodium succinate on children with allergic purpura(HSP)and to analyze the effect on cellular immune function.Methods A total of 150 children with HSP admitted to Hangzhou Children’s Hospital from February 2020 to February 2022.According to the principle of complete randomization,they were divided into observation group and the control group,75 cases in each group.The monotherapy group was treated by intravenous infusion of methylprednisolone sodium succinate,while the observation group was treated with vitamin D in addition to the monotherapy group.The differences of clinical efficacy,symptom disappearance,length of stay,cellular immune function and adverse reactions were analyzed between two groups.Results The total effective rate in observation group was higher than that in the control group(P<0.05).The disappearance time of rash,abdominal pain,joint swelling and pain and hospitalization days in the observation group were shorter than those in the control group(P<0.05).The levels of CD3+,CD4+and natural killer cells in the observation group were respectively higher than the the control group(P<0.05).There were no significant differences in the occurrence of adverse reactions between the two groups(P>0.05).Conclusion The combination of vitamin D and methylprednisolone sodium succinate has significant prognostic effect on children with HSP,which is beneficial to the improvement of cellular immune function and has good safety.
