摘要
目的研究类风湿关节炎(RA)患者血清25-羟维生素D水平与疾病活动度的相关性。方法选取60例类风湿关节炎患者作为观察组,根据28个关节疾病活动度评分(DAS28评分)分为高活动度组(DAS28评分>3.2分,5例)、中活动度组(DAS28评分2.6~3.2分,25例)及低活动度组(DAS28评分<2.6分,30例);另选取60例健康体检者作为对照组。检测两组血清25-羟维生素D水平,比较观察组和对照组的25-羟维生素D水平、不同疾病活动度患者血清25-羟维生素D水平,分析血清25-羟维生素D水平与疾病活动度的相关性。结果观察组血清25-羟维生素D(24.86±7.53)ng/ml低于对照组的(34.37±8.37)ng/ml,差异有统计学意义(P<0.05)。患者疾病活动度越高则血清25-羟维生素D水平越低,高活动度组血清25-羟维生素D(16.62±2.27)低于中活动度组的(22.43±4.47)ng/ml和低活动度组的(28.26±5.46)ng/ml,中活动度组血清25-羟维生素D水平低于低活动度组,差异有统计学意义(P<0.05)。患者25-羟维生素D水平与疾病活动度评分呈负相关(r=-0.251,P<0.05)。结论类风湿关节炎患者血清25-羟维生素D水平明显低于健康人,且与患者疾病活动度有明显关联。
Objective To study the correlation between serum 25-hydroxyvitamin D levels and disease activity in patients with rheumatoid arthritis(RA).Methods 60 patients with rheumatoid arthritis were selected as the observation group.According to 28-joint disease activity scores(DAS28 scores),they were divided into high activity group(DAS28 scores>3.2 points,5 cases),medium activity group(DAS28 scores 2.6-3.2 points,25 cases)and low activity group(DAS28 scores<2.6 points,30 cases);another 60 healthy subjects were selected as the control group.The serum 25-hydroxyvitamin D levels of the two groups were measured,and the serum 25-hydroxyvitamin D levels were compared between the observation group and the control group,as well as patients with different disease activity were compared.The correlation between serum 25-hydroxyvitamin D levels and disease activity was analyzed.Results The serum 25-hydroxyvitamin D in the observation group was(24.86±7.53)ng/ml,which was lower than(34.37±8.37)ng/ml in the control group,and the difference was statistically significant(P<0.05).The higher the disease activity,the lower the serum 25 hydroxyvitamin D level in patients.The serum 25 hydroxyvitamin D in the high activity group was(16.62±2.27),which was lower than(22.43±4.47)ng/ml in the medium activity group and(28.26±5.46)ng/ml in the low activity group;the serum 25 hydroxyvitamin D level in the medium activity group was lower than that in the low activity group;the differences were statistically significant(P<0.05).There was a negative correlation between 25 hydroxyvitamin D level and disease activity score(r=-0.251,P<0.05).Conclusion Serum 25 hydroxyvitamin D levels in patients with rheumatoid arthritis are significantly lower than those in healthy people,and are significantly related to disease activity.
