摘要
目的观察头痛宁胶囊联合阿司匹林肠溶片治疗偏头痛的效果,并探讨其对患者血管内皮功能和脑血流动力学的影响。方法回顾性分析2018年6月至2021年6月于延安大学附属医院治疗的82例偏头痛患者的临床资料,根据治疗方式的不同分组,其中39例采用阿司匹林肠溶片治疗者纳入对照组,43例采用头痛宁胶囊联合阿司匹林肠溶片治疗者纳入观察组。比较两组患者治疗后的临床疗效,治疗前后的血管内皮功能[内皮素(ET)、一氧化氮(NO)及5-羟色胺(5-HT)]、脑血流动力学[大脑前动脉(ACA)、大脑中动脉(MCA)、大脑后动脉(PCA)]及不良反应发生情况。结果观察组患者的治疗总有效率为95.35%,明显高于对照组的76.92%,差异有统计学意义(P<0.05);治疗后,观察组患者的ET、NO、5-HT水平分别为(36.41±7.59)μmol/L、(53.28±7.49)ng/mL、(436.25±32.59)ng/L,明显低于对照组的(57.36±10.65)μmol/L、(66.18±8.32)ng/mL、(551.48±38.72)ng/L,差异均有统计学意义(P<0.05);治疗后,观察组患者的ACA、MCA、PCA水平分别为(82.54±8.17)cm/s、(101.23±13.49)cm/s、(62.46±7.81)cm/s,明显低于对照组的(88.37±7.69)cm/s、(110.52±14.25)cm/s、(78.43±7.96)cm/s,差异均有统计学意义(P<0.05);观察组和对照组患者治疗期间的总不良反应发生率分别为11.63%,5.13%,差异无统计学意义(P>0.05)。结论头痛宁胶囊联合阿司匹林肠溶片治疗偏头痛疗效确切,能有效改善患者血管内皮功能和脑血流速度,且安全性尚可。
Objective To observe the effect of Tongtongning Capsule combined with Aspirin Enteric-coated Tablets in the treatment of migraine,and to explore its effect on vascular endothelial function and cerebral hemodynamics.Methods The clinical data of 82 patients with migraine who were treated in the Affiliated Hospital of Yan'an University from June 2018 to June 2021 were retrospectively analyzed.According to the different treatment methods,39 patients who were treated with Aspirin Enteric-coated Tablets were included in the control group,and 43 patients with Toutouning Capsule combined with Aspirin Enteric-coated Tablets were included in the observation group.The clinical efficacy after treatment,the vascular endothelial function[endothelin(ET),nitric oxide(NO),and 5-hydroxytryptamine(5-HT)],cerebral hemodynamics[anterior cerebral artery(ACA),middle cerebral artery(MCA),posterior cerebral artery(PCA)],and adverse reactions before and after treatment were compared between the two groups.Results The total effective rate in the observation group was 95.35%,which was significantly higher than 76.92%in the control group(P<0.05).After treatment,the levels of ET,NO,and 5-HT in the observation group were(36.41±7.59)μmol/L,(53.28±7.49)ng/mL,and(436.25±32.59)ng/L,respectively,which were significantly lower than(57.36±10.65)μmol/L,(66.18±8.32)ng/mL,(551.48±38.72)ng/L in the control group(P<0.05).After treatment,the levels of ACA,MCA,and PCA in the observation group were(82.54±8.17)cm/s,(101.23±13.49)cm/s,and(62.46±7.81)cm/s,respectively,which were significantly lower than(88.37±7.69)cm/s,(110.52±14.25)cm/s,(78.43±7.96)cm/s in the control group(P<0.05).The total incidence of adverse reactions in the observation group and the control group during treatment were 11.63%and 5.13%,respectively,and there was no significant difference between the two groups(P>0.05).Conclusion Toutongning Capsules combined with Aspirin Enteric-coated Tablets has definite curative effect in the treatment of migraine,which can effectively improve the vascular endothelial function and cerebral blood flow velocity in patients,with acceptable safety.
