摘要
胶粘类用品在医护领域应用广泛,就常规普通医用压敏粘胶材料的特性和不足,对皮肤产生的影响进行研究。实验主要针对儿童皮肤的临床应用及效果进行比较对照,该实验探究儿童医用粘胶相关性皮肤损伤(MARSI)危险因素,分析MARSI发生情况。试验分为MARSI组(23例)及非MARSI组(218例),比较2组临床资料,采用Logistic回归分析影响MARSI发生的危险因素。结果表明:241例中MARSI发生率为9.54%;张力性损伤的发生率为47.83%,高于表皮剥脱伤13.04%、过敏性皮炎17.39%、接触性皮炎13.04%、潮湿相关性皮肤损伤4.35%、毛囊炎4.35%,且差异显著(P<0.05);MARSI组中体重消瘦或肥胖、敷贴类型为普通胶布、住院时间大于等于7 d、前白蛋白小于100 mg/L、皮肤状态异常、消毒方案仅为碘伏、有皮肤过敏史的比例高于非MARSI组(P<0.05)。住院时间过长、前白蛋白低水平、皮肤状态异常、有皮肤过敏史等是影响儿童MARSI发生危险因素,可通过改善患儿皮肤状态等来降低MARSI发生率。
Adhesive products are widely used in the field of medical care.The characteristics and shortcomings of conventional medical pressure-sensitive adhesive materials,and their impact on the skin were studied.The clinical application and effect of children s skin were compared in the experiment.To investigate the risk factors of medical adhesive-related skin injury(MARSI),the occurrence of MARSI was analyzed.According to the occurrence of MARSI,the patients were divided into MARSI group(23 cases)and non-MARSI group(218 cases).The clinical data of the two groups were compared,and the risk factors affecting the occurrence of MARSI were analyzed by Logistic regression analysis.The results are as follows.The incidence rate of MARSI in 241 children was 9.54%.The incidence rate of tension injury was 47.83%,which was higher than that of epidermal exfoliation injury 13.04%,allergic dermatitis 17.39%,contact dermatitis 13.04%,moisture-related skin injury 4.35%and folliculitis 4.35%(P<0.05).The proportions of children who were thin or obese,applied with ordinary adhesive tape,with a hospital stay≥7 days,a prealbumin<100 mg/L,an abnormal skin condition,the disinfection regimen being only iodophor and a history of skin allergy,were higher in MARSI group than those in non-MARSI group(P<0.05).Long hospital stay,low level of prealbumin,abnormal skin condition,and presence of history of skin allergy are risk factors for the occurrence of MARSI in children.Improving the skin condition of children can reduce the incidence rate of MARSI.
