摘要
目的通过疾病诊断相关分组(diagnosis related groups,DRGs)对某院心血管外科进行医疗服务绩效评价,分析学科发展情况及并提出可行性建议。方法选取某院心血管外科2019年1月1日—2021年12月31日住院病案首页,使用DRGs进行分组,对心血管外科的医疗服务能力、效率和质量安全等指标进行对比分析。结果总权重(total weight,TW)数较2019增长6.74%,病例组合指数(case-mix index,CMI)增加0.11,出院人数保持稳定,床位使用率偏低,手术介入数、四级手术、微创手术例数2021年较2019分别增长9.7%,8.56%、26.73%,科室前3病种是周围血管手术和心脏瓣膜手术,时间消耗指数高于平均水平1,耗材费用持续增长,出现中低风险死亡病例。结论样本医院心血管外科医疗服务能力持续提升,心脏外科优势逐渐减弱,血管外科得到进一步发展,医疗服务效率和安全方面需要进一步提升。
Objective Through the diagnosis related groups(DRGs),the medical service performance of the cardiovascular surgery department of the sample hospital was evaluated,and the development of the discipline was analyzed and feasible suggestions were put forward.Methods The first page of inpatient medical records of cardiovascular surgery in a hospital from January 1,2019 to December 31,2021 was taken and grouped by DRGs,and the medical service capacity,efficiency,quality and safety of cardiovascular surgery were compared.analyze.Results Compared with 2019,the total weight(TW)increased by 6.74%,the case-mix index(CMI)increased by 0.11,the number of discharged patients remained stable,the utilization rate of beds was low,and the number of surgical intervention,grade 4 surgery and minimally invasive surgery increased by 9.7%,8.56%and 26.73%respectively in 2021 compared with 2019.The top three diseases in the department were peripheral vascular surgery and heart valve surgery.The time consumption index was higher than the average level 1,the cost of consumables continued to increase,and there were medium-low risk death cases.Conclusion The cardiovascular surgery medical service capacity of the sample hospitals continues to improve,the advantages of cardiac surgery are gradually weakened,and the vascular surgery has been further developed,and the efficiency and safety of medical services need to be further improved.
