摘要
患者,女,54岁。周身多发丘疹和结节5个月。皮肤科查体:全身散在粟粒至黄豆大的丘疹,其上可见白色鳞屑不易剥除,部分皮损表面可见菜花状改变;唇上缘可见一花生大红色结节,边界清晰,质地较硬,中央有“火山口”样溃疡,内含角质,表面可见毛细血管扩张。皮损组织病理示:鳞状上皮增生,棘细胞层增厚,角化亢进伴角化不全,表皮可见血痂角化层小灶,表皮突向下延伸,下端增宽呈杵状,真皮浅层毛细血管周围可见慢性炎细胞浸润。诊断:泛发性发疹性角化棘皮瘤(GEKA)。
A 54-year-old female presented with multiple papules and nodules all over her body for 5 months.Skin examination showed papules the size of a miliary to a soybean scattered all over the body,on which white scales are not easy to peel off,and cauliflower-like changes can be seen on the surface of some skin lesions;a red nodule of peanut size can be seen on the upper edge of the lip,with a clear boundary and hard texture,there is a"crater"-like ulcer in the center,containing keratinizing substance,and telangiectasia can be seen on the surface.The histopathology of the skin lesions showed hyperplasia of squamous epithelium,thickening of spinous cell layer,hyperkeratosis with incomplete keratosis,small focus of keratinized layer of blood scab in epidermis,downward extension of epidermal process and pestle-shaped widening at the lower end.chronic inflammatory cell infiltration can be seen around the capillaries of the superficial dermis.The diagnosis of generalized exanthematous keratoacanthoma(GEKA)was made.
