摘要
目的探究自拟小柴胡汤中药溻渍联合红外线理疗仪治疗风寒湿痹证类风湿关节炎的效果。方法采用随机数字表法将驻马店市中医院2020年5月至2022年3月风湿免疫科收治96例类风湿性关节炎患者分为两组,各48例患者。对照组接受来氟米特片治疗,观察组在此基础上联合使用自拟小柴胡汤中药溻渍联合红外线理疗仪治疗。观察两组患者治疗前后炎症因子[C反应蛋白(CRP)、白细胞介素-1(IL-1)、白细胞介素-6(IL-6)]水平变化与红细胞沉降率(ESR)、类风湿因子(RF)水平变化及疼痛程度、疾病活动度[视觉模拟评分法(VAS)、活动度评价表(DAS28)、简化活动指数(SDAI)]评分变化。比较两组患者临床疗效及不良反应发生率。结果两组患者治疗后观察组临床总有效率(93.76%)高于对照组(79.20%)(χ^(2)=4.359,P=0.036)。治疗前两组患者CRP、IL-1、IL-6水平差异无统计学意义(P>0.05),治疗后观察组CRP、IL-1、IL-6水平较对照组降低(P<0.05)。治疗前两组患者ESR、RF水平差异无统计学意义(P>0.05),治疗后观察组ESR、RF水平较对照组降低(P<0.05)。两组患者治疗前VAS、DAS28、SDAI评分差异无统计学意义(P>0.05),治疗后观察组VAS、DAS28、SDAI评分较对照组降低(P<0.05)。观察组不良反应发生率(6.24%)略低于对照组(8.32%),差异无统计学意义(χ^(2)=0.154,P=0.694)。结论自拟小柴胡汤中药溻渍联合红外线理疗仪能有效降低类风湿关节炎患者炎症因子水平,缓解患者痛苦,促进关节功能的恢复,降低不良反应发生率,提升临床疗效。
Objective To investigate the clinical efficacy of TCM dip-soaking therapy with self made Xiaochaihu decoction combined with infrared therapy apparatus in treating rheumatoid arthritis of the wind cold dampness arthralgia.Methods Ninety-six patients with rheumatoid arthritis in Rheumatology and Immunology Department of Zhumadian Hospital of Traditional Chinese Medicine from May 2020 to March 2022 were enrolled,and randomly classified into two groups,each with 48 cases.Control group received leflunomide treatment,based on this,observation group received TCM dip-soaking therapy with self made Xiaochaihu decoction combined with infrared therapy apparatus.The levels of inflammation factors[C-reactive protein(CRP),interleukin-1(IL-1)and IL-6],erythrocyte sedimentation rate(ESR),rheumatoid factor(RF)and pain level,disease activity level[visual analogue scale(VAS),disease activity score 28(DAS28),simplified disease activity index(SDAI)]were observed between two groups before and after treatment.Clinical efficacy and adverse reaction rate were compared between two groups.Results The overall efficacy rate was higher than that in control group(93.76%vs.79.20%,χ^(2)=4.359,P=0.036).Serum levels of CRP,IL-1 and IL-6 at baseline yielded no statistical difference between two groups,while the three inflammation factors were decreased in observation group compared with control group after treatment(P<0.05).ESR and RF values demonstrated no statistical difference between two groups before treatment(P>0.05),while the two indicators of observation group were significantly lower than those of control group after treatment(P<0.05).Scores of VAS,DAS28 and SDAI denoted no statistical difference between two groups before treatment(P>0.05).The post-treatment VAS,DAS28 and SDAI scores of observation group were decreased compared with control group(P<0.05).The adverse reaction rate was lower than that in control group,with no statistical difference(6.24%vs.8.32%,χ^(2)=0.154,P=0.694).Conclusion Application of TCM dip-soaking therapy with self made Xiaochaihu decoction combined with infrared therapy apparatus in treating rheumatoid arthritis of the wind cold dampness arthralgia can effectively attenuate inflammation reaction,alleviate pain,promote the recovery of joint function and reduce the adverse reactions.
