摘要
目的 探讨穴位按摩在预防机械通气肠内营养支持患者腹胀中的应用效果。方法 选取2022年4月28日至2022年8月31日在我院住院的机械通气肠内营养支持患者100例为研究对象,采用抛硬币法随机分成对照组和观察组,每组50例。对照组实施肠内营养支持后行常规护理,观察组采用穴位按摩下机械通气并肠内营养支持的护理程序。比较两组临床疗效、胃肠道症状,护理前、护理后(肠内营养支持后7 d)的胃残余量、腹围、腹内压、胃肠功能障碍评分。结果 与对照组(80.00%)相比,观察组护理总有效率(92.00%)更高(χ~2=5.181,P=0.023);护理后观察组与对照组24 h胃残余量、腹围、腹内压均下降,且观察组24 h胃残余量、腹围、腹内压低于对照组(P <0.05);护理后观察组与对照组胃肠功能障碍评分下降,且观察组胃肠功能障碍评分低于对照组(P <0.05);观察组便秘、呕吐、肠鸣音减弱或消失、腹泻发生率均低于对照组(P <0.05)。结论 穴位按摩可有效改善行肠内营养支持的人工气道机械通气患者的胃肠功能,减少胃残余量,降低腹围及腹内压,减少腹胀发生风险。
Objective To explore the effect of acupoint massage in preventing abdominal distension in patients with mechanical ventilation and enteral nutrition support.Methods A total of 100 patients with mechanical ventilation and enteral nutrition support who were hospitalized in our hospital from April 28,2022 to August 31,2022 were selected as the research objects,the coin toss method was used to randomly divide the control group and the observation group,with 50 cases in each group.In the control group,routine care was performed after the implementation of enteral nutrition support,and the observation group received the nursing procedure of mechanical ventilation under acupoint massage and enteral nutrition support.The clinical efficacy,gastrointestinal symptoms,gastric residual volume,abdominal circumference,intra-abdominal pressure,and gastrointestinal dysfunction scores before and after nursing(7 d after enteral nutrition support) were compared among two groups.Results The total effective rate of nursing in observation group(92.00%) was higher when compared with the control group(80.00%)(χ~2=5.181,P=0.023).After nursing,the 24-hour gastric residual volume,abdominal circumference and intra-abdominal pressure in the observation group and the control group decreased,and the 24-hour gastric residual volume,abdominal circumference and intra-abdominal pressure in the observation group were lower when compared with the control group(P<0.05).The gastrointestinal dysfunction score in the two groups decreased,and the gastrointestinal dysfunction score in the observation group was lower when compared with the control group(P<0.05).The incidence of constipation,vomiting,bowel sounds weakened or disappeared,and diarrhea in the observation group were lower when compared with the control group(P<0.05).Conclusion Acupoint massage can effectively improve the gastrointestinal function of patients undergoing enteral nutrition support with artificial airway and mechanical ventilation,reduce gastric residual volume,reduce abdominal circumference and intra-abdominal pressure,and reduce the risk of abdominal distension.
