摘要
目的比较腹腔镜经腹腹膜前疝修补术(TAPP)与开放式手术治疗成人腹股沟疝的临床效果。方法选取我院收治的100例成人腹股沟疝患者作为研究对象,利用计算机数字随机模型将其分为对照组(n=50,开放式无张力疝修补术)和观察组(n=50,TAPP)。比较两组的治疗效果。结果观察组的手术时间长于对照组,术中出血量少于对照组,肛门恢复排气时间、术后下床活动时间、总住院时间短于对照组(P<0.05)。观察组的术后并发症总发生率低于对照组(P<0.05)。术后1、3、5 d,观察组的数字分级法(NRS)评分低于对照组(P<0.05)。观察组的对侧隐匿疝发现率高于对照组(P<0.05)。术后5 d,两组的肿瘤坏死因子-α(TNF-α)、C反应蛋白(CRP)、白细胞介素-6(IL-6)水平均升高,但观察组低于对照组(P<0.05)。结论较传统开放式手术,TAPP对成人腹股沟疝患者的创伤更小,术后恢复更快,手术并发症发生率更低,使患者能够尽早回归健康生活,值得临床推广应用。
Objective To compare the clinical effects between laparoscopic transabdominal preperitoneal hernia repair(TAPP)and open surgery in the treatment of adult inguinal hernia.Methods A total of 100 adult inguinal hernia patients admitted in our hospital were selected as the research object and divided into control group(n=50,open tension-free hernia repair)and observation group(n=50,TAPP)by computer numerical random model.The therapeutic effect of the two groups was compared.Results The operation time in the observation group was longer than that in the control group,the amount of intraoperative bleeding was less than that in the control group,and the time of anal recovery and exhaust,the time of getting out of bed after operation and the total hospital stay were shorter than those in the control group(P<0.05).The total incidence of postoperative complications in the observation group was lower than that in the control group(P<0.05).At 1,3,5 d after operation,the Numerical Rating Scale(NRS)score in the observation group was lower than that in the control group(P<0.05).The detection rate of contralateral occult hernia in the observation group was higher than that in the control group(P<0.05).At 5 d after operation,the levels of tumor necrosis factor-α(TNF-α),C-reactive protein(CRP)and interleukin-6(IL-6)in both groups increased,but those in the observation group were lower than the control group(P<0.05).Conclusion Compared with traditional open surgery,TAPP has less trauma to adult inguinal hernia patients,faster postoperative recovery and lower incidence of surgical complications,so that patients can return to healthy life as soon as possible,which is worthy of clinical promotion and application.
