妊娠早中期超声指标在单绒毛膜双胎中的临床研究进展 被引量:1

Clinical progress of first and second trimester ultrasound indexes in monochorionic twins

摘要 近年来,高龄孕妇增加伴随辅助生殖技术在临床工作中广泛应用,使双胎妊娠比例上升。据北京协和医院统计,1990年至2014年双胎妊娠胎儿总数占围产儿总数的比例由2.0%上升至5.0%[1]。美国统计资料显示,近10年双胎妊娠约占活产婴儿的3.2%~3.3%,较1980年增加76%。单卵双胎的发生率相对稳定,约为(3~5)/1000,其中单绒毛膜双羊膜囊双胎(monochorionic-diamniotic,MCDA)占单卵双胎的68%,单绒毛膜单羊膜囊双胎(monochorionicmonoamniotic,MCMA)占单卵双胎的1%~2%[2],其余为双绒毛膜双胎。与双绒毛膜双胎相比,单绒毛膜双胎胎儿畸形发生率增高2~3倍,并且具有双胎输血综合征(twinto-twin transfusion syndrome,TTTS)、双胎贫血-红细胞增多序列征(twin anemia-polycythemia sequence,TAPS)、选择性胎儿生长受限(selective fetal growth restriction,sFGR)、双胎反向动脉灌注(twin reversed arterial perfusion,TRAP)序列征、联体双胎等特殊并发症,胎儿及围产儿发病率、死亡率均增加[3]。
