摘要
急性呼吸窘迫综合征(ARDS)是一种由于严重休克、创伤、感染以及烧伤引起的以进行性加重的低氧血症、呼吸窘迫为主要临床表现的疾病,是常见的危重症之一,患病后病死率高。中医归为“暴喘”的范畴,其病位主要在肺。《灵枢·九针论》中记载“手阳明太阴为表里”,提出了肺与大肠相表里的理论。肺与大肠经脉相互属络,在生理上相互作用,病理上相互影响。肺的肃降功能有助于大肠传导功能的运行,大肠传导功能正常运行也有利于肺气肃降。基于“肺与大肠相表里”理论,探讨采用“肺肠同治法”,运用宣白承气汤治疗急性呼吸窘迫综合征的理论机制,并结合临床案例,以期为ARDS的治疗提供理论依据及临床思路。
Acute respiratory distress syndrome(ARDS)is a disease with progressive aggravation of hypoxemia and respiratory distress as the main clinical manifestations due to severe shock,trauma,infection and burn.In traditional Chinese medicine,this disease can be classified into the category of"dyspnea",and its location is mainly in lung.The theory that lung and large intestine are exterior-interiorly related was proposed based on the record of"Hand Yangming’s Taiyin being exterior-interiorly related"in Miraculous Pivot Nine-needle Theory.The meridians of lung and large intestine interconnect and interact physiologically and pathologically.The descending function of lung is conducive to the operation of large intestinal conduction function,and the normal operation of large intestinal conduction function is also conducive to the descending of lung Qi.Based on the theory of"exterior-interior relationship between lung and large intestine",this paper discussed the theoretical mechanism of the treatment of acute respiratory distress syndrome with the method of"lung-intestine co-treatment"and Xuanbai Chengqi Decoction(宣白承气汤)and combined with clinical cases,in order to provide a theoretical basis and clinical ideas for the treatment of ARDS.
