摘要
目的探讨腹腔镜下胃癌根治术治疗进展期胃癌患者的近期疗效及对其免疫功能与远期预后的影响。方法选取2016年1月至2019年6月莆田学院附属医院收治的80例进展期胃癌患者为研究对象,根据患者自愿选择的术式不同分为对照组和试验组,各40例。对照组采用传统开腹手术治疗,试验组采用腹腔镜下胃癌根治术治疗,比较两组手术指标、手术前后T淋巴细胞含量[包括CD3^(+)、CD4^(+)、CD4^(+)/CD8^(+)]、术后并发症发生情况及远期预后。结果试验组手术时间长于对照组,住院时间及切口长度均短于对照组,术中出血量少于对照组,差异有统计学意义(P<0.05);术前,两组CD3^(+)、CD4^(+)及CD4^(+)/CD8^(+)比较,差异无统计学意义(P>0.05);术后,两组CD3^(+)、CD4^(+)及CD4^(+)/CD8^(+)均低于术前,但试验组术后CD3^(+)、CD4^(+)及CD4^(+)/CD8^(+)均高于对照组,差异有统计学意义(P<0.05);试验组术后并发症总发生率低于对照组,差异有统计学意义(P<0.05);两组术后3年生存率与复发率比较,差异无统计学意义(P>0.05)。结论腹腔镜下胃癌根治术治疗进展期胃癌患者的疗效确切,患者术中出血量少、切口小、住院时间短,远期预后良好,且该手术对患者免疫功能的抑制较轻,术后并发症发生率较低。
Objective To observe the short-term efficacy of laparoscopic radical gastrectomy for advanced gastric cancer,and its influence on immune function and long-term prognosis.Methods A total of 80 patients with advanced gastric cancer admitted to the hospital from January 2016 to June 2019 were selected,and divided into control group and laparoscopic group according to the surgical method chosen by themselves,with 40 patients in each group.Patients in the control group were treated with traditional laparotomy,and those in the laparoscopic group were treated with laparoscopic radical gastrectomy.Surgical indicators,cellular immune function(CD3^(+),CD4^(+)and CD4^(+)/CD8^(+)),the incidence of complications and long-term prognosis were compared between the groups.Results The bleeding volume,incision and hospital stay of the laparoscopic group were significantly smaller/shorter than those of the control group(P<0.05).After operation,the levels of CD3^(+),CD4^(+)and CD4^(+)/CD8^(+)were decreased in the two groups,and significantly higher in the laparoscopic group than in the control group(P<0.05).The total incidence of complications in the laparoscopic group(5.00%)was significantly lower than that in the control group(22.50%)(P<0.05).The 3-year survival rate and recurrence rate in the laparoscopic group were slightly higher than those in the control group(P>0.05).Conclusions Laparoscopic radical gastrectomy is effective in the treatment of advanced gastric cancer.It has the characteristics of little blood loss,small incision,and short hospital stay.Meanwhile,it can significantly relieve immune suppression after operation,with a low incidence of complications and better long-term prognosis.
