摘要
目的 探讨调强放疗联合替吉奥加顺铂(SP)方案化疗治疗老年局部晚期食管鳞癌患者的疗效和安全性。方法 选取安阳市肿瘤医院2018年1月至2020年1月收治的92例老年局部晚期食管鳞癌患者,随机均分为研究组和对照组2组,每组46例。对照组采取SP化疗方案,研究组在对照组基础上联合调强放疗。比较观察2组近期疗效、生活质量、不良反应发生情况等。结果 研究组疾病控制率为84.78%,高于对照组的65.22%,差异有统计学意义(χ~2=4.696,P=0.030)。治疗后,研究组和对照组KPS评分较治疗前高(t=18.174,P<0.001;t=12.296,P<0.001),且研究组高于对照组(t=5.768,P<0.001)。2组肾肝功能损伤、血小板减少、粒细胞减少、胃肠道反应、骨髓抑制发生率较低,程度较轻,未影响治疗的顺利进行。结论 采取调强放疗联合SP方案化疗治疗老年局部晚期食管鳞癌患者,可有效提升肿瘤控制率,改善患者生活质量,且安全性良好。
Objective To investigate the efficacy and safety of intensity modulated radiotherapy combined with S-1 plus cisplatin regimen chemotherapy in the treatment of elderly patients with locally advanced esophageal squamous cell carcinoma. Methods All the 92 elderly patients with locally advanced esophageal squamous cell carcinoma in Anyang Cancer Hospital from January 2018 to January 2020 were randomly divided into the two groups. The control group was treated with SP chemotherapy,and the study group was treated with intensity modulated radiotherapy on the basis of the control group. The short-term efficacy,quality of life and side effects were analyzed and compared between the two groups. Results The disease control rate in the study group( 84. 78%) was higher than that in the control group( 65. 22%),with a statistically significant difference( χ2= 4. 696,P = 0. 030). After treatment,the KPS scores in the study group and the control group were higher than those before treatment( t = 18. 174,P < 0. 001;t = 12. 296,P < 0. 001),and the KPS scores in the study group was higher than that in the control group( t = 5. 768,P < 0. 001). The incidence and degree of renal and liver function injury,thrombocytopenia,granulocytopenia,gastrointestinal reaction and bone marrow suppression in the two groups were low and mild,which did not affect the smooth progress of treatment. Conclusion Intensity modulated radiation therapy combined with SP chemotherapy in the treatment of elderly patients with locally advanced esophageal squamous cell carcinoma can effectively improve the tumor control rate,improve the quality of life of patients,and have the good safety.
