摘要
目的探讨2018-2021年某院抗菌药物使用强度与耐利奈唑胺肠球菌检出率的动态相关性,为抗菌药物应用管理和合理使用提供参考。方法回顾性统计分析2018-2021年我院粪肠球菌和屎肠球菌检出率、耐药率及抗菌药物使用强度,应用Spearman相关法对抗菌药物使用强度与耐利奈唑胺肠球菌检出率进行相关性分析。结果抗菌药物使用强度总体呈上升趋势,以β-内酰胺酶抑制剂复方制剂、三代头孢和碳青霉烯类增长较快。粪肠球菌对大部分抗菌药物耐药率均显著低于屎肠球菌。耐利奈唑胺粪肠球菌检出率与同期万古霉素和利奈唑胺使用强度呈正相关;耐利奈唑胺屎肠球菌检出率与6个月前四代头孢使用强度呈正相关。结论耐利奈唑胺肠球菌检出率与抗菌药物使用强度存在动态相关性,应持续加强抗菌药物管理,减少细菌耐药。
Objective To explore the dynamic correlations between the usage of antimicrobials and the detection rate of linezolid-resistant Enterococci(LRE)in a hospital in order to provide reference for the control and rational use of antimicrobials.Methods The detection rate,drug resistance rate of Enterococcus faecalis and Enterococcus faecium and the usage of antimicrobials in our hospital between 2018 and 2021 were retrospectively analyzed.The Spearman correlation method was used to analyze the correlations between the usage of antibiotics and the detection rate of LRE.Results The overall usage of antibacterial drugs was on the rise,especiallyβ-lactamase inhibitor compound preparations,third-generation cephalosporins and carbapenems.The rate of resistance of Enterococcus faecalis to most antibiotics was significantly lower than that of Enterococcus faecium.The detection rate of linezolid-resistant Enterococcus faecalis was positively correlated with the usage of vancomycin and linezolid in the same period.The detection rate of linezolid-resistant Enterococcus faecium was positively correlated with the amount of fourthgeneration cephalosporins used six months before.Conclusion There is a dynamic correlation between the detection rate of LRE and the usage of antimicrobials,making antimicrobial control necessary to reduce bacterial resistance.
