摘要
目的:通过分析21例急性闭合性跟腱断裂的MRI表现,探讨3.0T磁共振成像在急性闭合性跟腱断裂中的诊断价值。方法:选取北大医疗鲁中医院骨科2017年7月—2022年7月收治的21例跟腱断裂且行3.0T MRI扫描的患者为研究对象,对21例急性闭合性跟腱断裂的临床及影像资料进行回顾与分析。结果:21例急性闭合性跟腱断裂患者中,跟腱上段断裂3例,跟腱中段断裂14例,跟腱下段断裂4例。其中完全跟腱断裂19例,不完全断裂2例。完全跟腱断裂MRI信号表现为跟腱不连续,断裂部位不规则T_(1)WI低信号,FSPDWI高信号,FS-T_(2)WI高信号;两断端挛缩。不完全跟腱断裂MRI表现为跟腱部分不连续,可见线样T_(1)WI低信号,FS-PDWI高信号,FS-T_(2)WI高信号,异常信号未达跟腱全部宽度。结论:3.0T磁共振能够清晰显示跟腱断裂情况并且能够多方位扫描观察跟腱断裂情况,有利于疾病的早诊断、早治疗。
Objective To explore the diagnostic value of 3.0T magnetic resonance imaging in acute closed achilles tendon rupture,by analyzing MRI features of 21 cases of acute closed Achilles tendon rupture.Methods A total of 21 patients with Achilles tendon rupture who underwent 3.0T MRI scan in Department of Orthopedics,PKUCare Luzhong Hospital from July 2017 to July 2022 were selected as the research objects.The clinical and imaging data of 21 patients with acute closed Achilles tendon rupture were reviewed and analyzed.Rusults Among the 21 patients with acute closed Achilles tendon rupture,there were 3 cases of upper Achilles tendon rupture,14 cases of middle Achilles tendon rupture and 4 cases of lower Achilles tendon rupture.There were 19 cases of complete rupture and 2 cases of incomplete rupture.The MRI signal of complete Achilles tendon rupture showed that the Achilles tendon was discontinuous,and the irregular rupture site had low signal on T_(1)WI,high signal on FS-PDWI,and high signal on FS-T_(2)WI.Contracture of two broken ends.MRI of incomplete Achilles tendon rupture showed partial discontinuity of the Achilles tendon,with low signal intensity on linear T_(1)WI,high signal intensity on FS-PDWI,and high signal intensity on FS-T_(2)WI.Abnormal signals did not reach the full width of the Achilles tendon.Conclusion 3.0T magnetic resonance imaging can clearly show the condition of Achilles tendon rupture and observe the condition of Achilles tendon rupture by multi-directional scanning,which is conducive to the early diagnosis and treatment of the disease.
