摘要
目的研究双排螺旋CT在鉴别中央型肺癌与炎性肿块中的应用价值。方法抽选本院2019年7月-2021年9月肺部肿块患者62例,根据病理检查结果,将确诊为中央型肺癌患者33例设为恶性组,将确诊为炎性肿块患者29例设为良性组,对两组均行双排螺旋CT,比较两组CT征象分布、平均通过时间(MTT)、血流量(BF)、血容量(BV)、表面通透性(PS),观察双排螺旋CT鉴别诊断中央型肺癌与炎性肿块的敏感度、特异度、准确性、阳性预测值、阴性预测值及Kappa值。结果CT图像显示,炎性肿块呈现延迟强化表现,而中央型肺癌呈现快进快出表现;两组刀切征、分叶征及毛刺征分布比较,差异有统计学意义(P<0.05);两组其余征像分布比较,差异无统计学意义(P>0.05);两组MTT比较,差异无统计学意义(P>0.05),恶性组BF、BV、PS值均小于良性组,差异有统计学意义(P<0.05);双排螺旋CT鉴别诊断中央型肺癌与炎性肿块的敏感度为87.88%,特异度为89.66%,准确度为88.71%,阳性预测值为90.63%,阴性预测值为86.67%,Kappa值为0.774。结论双排螺旋CT有助于鉴别诊断中央型肺癌与炎性肿块,值得临床应用。
Objective To study the application value of double-slice spiral CT in the differential diagnosis of central lung cancer and inflammatory masses.Methods A total of 62 patients with lung masses from July 2019 to September 2021 in our hospital were selected.According to the results of pathological examination,33 patients diagnosed with central lung cancer were set as malignant group,and 29 patients diagnosed with inflammatory masses were set as benign group.Double-slice spiral CT was performed in both groups.The distribution of CT signs,mean transit time(MTT),blood flow(BF),blood volume(BV)and surface permeability(PS)were compared between the two groups.The sensitivity,specificity,accuracy,positive predictive value,negative predictive value and Kappa value of double-slice spiral CT in the differential diagnosis of central lung cancer and inflammatory masses were observed.Results CT images showed that inflammatory masses showed delayed enhancement,while central lung cancer showed fast in and fast out.There were significant differences in the distribution of knife-cut sign,lobulation sign and burr sign between the two groups(P<0.05).There was no significant difference in the distribution of other signs between the two groups(P>0.05).There was no significant difference in MTT between the two groups(P>0.05).The BF,BV and PS values of the malignant group were smaller than those of the benign group,and the difference was statistically significant(P<0.05).The sensitivity,specificity,accuracy,positive predictive value,negative predictive value and Kappa value of dual-slice spiral CT in differential diagnosis of central lung cancer and inflammatory masses were 87.88%,89.66%,88.71%,90.63%,86.67%and 0.774,respectively.Conclusion Dual-slice spiral CT is helpful in the differential diagnosis of central lung cancer and inflammatory masses,which is worthy of clinical application.
