摘要
目的探讨肺结核患者血清中微小RNA-431(miR-431)、微小RNA-194-5p(miR-194-5p)、细胞因子信号转导抑制物1(SOCS1)的表达及用于诊断肺结核的效能。方法选取2019年11月至2020年11月台州市第一人民医院收治的肺结核患者45例(肺结核组)、肺炎患者45例(肺炎组)、肺癌患者45例(肺癌组)及同期在本院体检中心健康体检的健康志愿者45名(对照组)作为研究对象。采用RT-PCR法检测所有研究对象入院时空腹血清miR-431、miR-194-5p表达水平,采用ELISA法检测血清SOCS1表达水平。ROC曲线评估血清miR-431、miR-194-5p、SOCS1单独检测及联合检测诊断肺结核的效能。采用Pearson相关分析miR-431、miR-194-5p、SOCS1表达水平的相关性。结果肺结核组患者血清miR-431、miR-194-5p表达水平均高于对照组和肺癌组,低于肺炎组;SOCS1表达水平低于对照组和肺癌组,高于肺炎组,差异均有统计学意义(均P<0.05)。有吸烟史、有结核空洞、病灶范围大、疾病处于进展期与肺结核患者血清miR-431、miR-194-5p、SOCS1表达水平均有关(均P<0.05)。ROC曲线显示,与血清miR-431、miR-194-5p、SOCS1单独检测以及miR-431联合miR-194-5p、miR-431联合SOCS1、miR-194-5p联合SOCS1检测相比,3项指标联合检测对肺结核的诊断效能更高(均P<0.05)。相关性分析显示,肺结核患者血清miR-431表达水平和miR-194-5p表达水平呈正相关(r=0.680,P<0.01),血清miR-431、miR-194-5p表达水平与SOCS1表达水平均呈负相关(r=-0.962、-0.639,均P<0.01)。结论肺结核患者血清中miR-431、miR-194-5p表达升高,SOCS1表达降低,3项指标之间呈线性相关,且与患者的临床病理特征有关,对肺结核的临床诊断有参考价值。
Objective To investigate the expression and diagnostic value of serum microRNA 431(miR 431),microRNA 194-5p(miR 194-5p),suppressor of cytokine signaling 1(SOCS1)in patients with pulmonary tuberculosis.Methods Forty-five patients with pulmonary tuberculosis(pulmonary tuberculosis group),45 patients with pneumonia(pneumonia group)and 45 patients with lung cancer(lung cancer group)admitted in Taizhou First People's Hospital from November 2019 to November 2020 were enrolled in the study;and 45 healthy subjects served as normal control group.The RT-PCR was used to detect serum miR 431 and miR-194-5p,and the enzyme linked immunosorbent assay(ELISA)was used to detect serum SOCS1.The receiver operating characteristic curve(ROC)was used to analyze the diagnostic value of miR-431,miR-194-5p,SOCS1 in pulmonary tuberculosis.Pearson correlation was used to analyze the correlation between miR-431,miR-194-5p and SOCS1 expression levels.Results The expression levels of serum miR-431 and miR-194-5p in the pulmonary tuberculosis group were higher than those in the normal control group and lung cancer group,but lower than those in the pneumonia group(all P<0.05).The expression of serum miR 431,miR 194-5p and SOCS1 was significantly associated with history of smoking,tuberculosis cavity,more lung fields occupied by lesions and advanced disease in patients with pulmonary tuberculosis(all P<0.05).The ROC showed that compared with the single detection of serum miR 431,miR 194-5p or SOCS1,and the combined detection of miR 31 and miR 194-5p,the combined detection of miR-431 and SOCS1,and the combined detection of miR-194-5p and SOCS1,the combined detection of 3 indicators showed advantage in diagnosing of pulmonary tuberculosis(all P<0.05).The expression of serum miR-431 was positively correlated with serum miR 194-5p(r=0.680,P<0.01),negatively correlated with SOCS1(r=-0.962,P<0.01);and the expression of miR 194-5p was negatively correlated with SOCS1(r=-0.639,P<0.01).Conclusion The expressions of serum miR-431 and miR-194-5p in pulmonary tuberculosis are increased,and the expression of SOCS1 is decreased.The expressions of miR 431,miR 194-5p and SOCS1 are related to the clinicopathological characteristics of patients,which may have diagnostic value for patients with pulmonary tuberculosis.
